If you could benefit from a rhinoplasty I’m sure you can find a surgeon who will take care of you. But every case is different. You mention body dysmorphia but that is a very vague statement

It can be done with only tipplasty with closed approach if you want natural result. I agree with you about hanging columella is the most problem of your nose
