Hello, and thank you for your question regarding an 80-pound weight loss and whether a crescent or spiral thigh lift is an option for you. Thank you for your history and your picture. Based on your pictures, you have excess thigh skin and fat that extends from the groin to approximately midway down your upper inner thigh. A crescent or a spiral thigh lift will help, but these two thigh lift options would not be your best thigh reduction surgery. The crescent or the spiral thigh lift will not significantly reduce your inner thigh skin and fat. Also, the crescent and the spiral thigh lift have significant risks, including scar migration. That means the crescent thigh lift scar can migrate from the groin out beyond the panty line. If this crescent thigh scar migration occurs, the crescent thigh scar can become quite visible on the anterior portion of the thigh. Also, asd the crescent or spiral thigh lift migrates, the scar can lead to labial spread and opening of the introitus. The introitus is the entrance to the vagina. That means if performed incorrectly, the crescent thigh lift can lead to gaping of the introitus or vagina. This complication is a very difficult surgery to correct. Therefore, the better surgery for you would be a modified longitudinal or vertical medial thighplasty. During a modified longitudinal, vertical medial thigh reduction, thigh liposuction is performed, and an incision is made from the groin partially down your inner thigh. The incision would extend to approximately halfway down your inner thigh, and the excess thigh skin and fat would be removed. This would be your best thigh surgery. I hope this helps. I hope that I have answered your question regarding whether the crescent or the spiral thigh lifts are options for you. Most plastic surgeons suggest you go with the best thigh surgery and not necessarily the thigh lift surgery, where you can best hide the scar. Good luck. I hope that I have answered your questions regarding the thigh reduction. If you have any more questions regarding the thigh lift, thigh reduction, vertical thigh lift, medial thigh lift, spiral thigh lift, crescent thigh lift, or thigh lipo, contact an experienced plastic surgeon. Please seek an experienced, board-certified plastic surgeon with significant experience performing thigh lifts. Do your research. Make sure your chosen plastic surgeon has performed at least 100 thigh lifts. Make sure they have the before and after pictures to prove it. Carefully examine their before and after pictures and check their reviews on RealSelf. Sincerely, Dr. Katzen, MD, MBA.
