Are tissue expanders always required?

I will be having a bilateral mastectomy as a preventive measure due to several risk factors. I am a candidate for nipple and skin sparing surgery with immediate reconstruction. I have d size breast now and would like implants that will be the same as possible. With the amount of skin I currently have, is it a given that I will still need expanders? Thank you!
