Any idea what this might be? (Photo)

I'm 2 1/2 yrs post reconstruction surgery. High profile, under muscle. This side has been bothering me with pain sometimes, shift in placement, and harder. Now I have this. I have no nipples, no nipple reconstructed. It's not itching. A bit of soreness in armpit muscles. In foreign country right now, not close to doctor. What could this be? It came rather quickly

Usually not but would check with your surgeon to see in person... very rarely is there a mechanical issue with the tissue expander

I definitely agree with ultrasound of the breasts in this setting. 5 months is quite a long time to be on antibiotics. Perhaps the remaining skin is just edematous (swollen) and there is no underlying bacterial infection. Follow up with your surgeon and let them know your concerns.

Depending on the location of the pectoralis muscle (most commonly) over the implant loose skin can be adjusted or contoured during the process of breast reconstruction- physical therapy probably didn't cause the loose skin but would check with your plastic surgeon..

May an ice bag wrapped in a towel be placed on swelling 6 weeks postoperative mastectomy with DEIP for aching?

In the area of the mastectomy skin (native skin) the sensation is different, and so indeed any ice or heating pack to the area might potentially cause a burn in the area and so would be careful- alternatively I am not sure ice will change the underlying sensation and so would ask your plastic...
