Droopy Right Eye Five Months AFTER Upper Eye Blepharoplasty Surgery...what Can Be Done? (photo)

Question

I had blepharoplasty surgery five months ago and my eyes, especially the right look worse than before. The right eye especially seems difficult to open as widely as I once could and it is now smaller than the left eye which is wasn't before. Has a muscle been cut or is it swelling or nerve damage? Can anything be done? I am including pictures

This is actually a pretty common situation. To be fair, persistent swelling is enough to cause a droopy eyelid after surgery as well. However, it is very common for me to enter an eyelid to fix someone who had eyelid surgery elsewhere and find the levator aponeurosis has been cleanly cut off from the tarsus. Mechanically, then the doctor pulls up the skin and muscle at the time of blepharoplasty the levator also lifts up and is easily cut. Generally there is actually little consequence from this due to the lateral extensions of the levator remaining intact. However, for some this is the precipitating event for the development of ptosis. The situation is generally covered in the surgical consent and fixable with surgery.

It appears you have upper lid ptosis, which is usually due to relaxation of the muscle responsible for lifting the upper eyelids. Upper blepharoplasty only removes excess loose skin from the upper eyelids; it does not actually lift the lids. You may benefit from upper lid ptosis surgery. See an oculoplastic surgeon for evaluation.

You probably had developed ptosis in your eyelid. If it is not improving with time, you may be a candidate for ptosis surgery. I would recommend having you see an oculoplastic surgeon for eyelid evaluation. Good luck.
