It is also possible that "all on X" is all that is really needed, and the jaw advancement can be avoided. I would suggest talking to surgeons that are experienced in each modality and after talking to a few, decide the correct approach.

Depending upon the severity of the underbite, the all on four dental implants procedure could help the situation.  If you have many failing teeth on the upper and lower jaws, then when the All on four is performed the bite relationship between the upper and lower jaws could be improved.  Ideally if just one surgery could fix the failing teeth and the underbite, that would be the best option.
