Question

I have these fatty areas on my face that won’t go away. They seem to be unaffected by how much I weigh because they didn’t go away when I was underweight, overweight, or normal weight. The fat has been there since I was a child. It’s most noticeable when I smile.

It appears that the peri-oral mounds that you are concerned about are the very location of the buccal fat pads.  This is unlikely to change with weight fluctuations and will not go away, even with significant weight loss.  You would be a great candidate for buccal fat removal.  The procedure is done through incisions inside your mouth so that there are no visible scars.

Thanks for the question and sharing your images. You have excess fat in the area around your lips. This is in front of the typical area that is addressed by removal of the buccal fat pad. Therefore the best option would be carefully and delicately performed perioral liposuction focusing on your areas of concern. This should be performed quite conservatively to avoid an overdone or unnatural appearance.
