Can corset platysmaplasty cause submandibular glands to fall when well-suspended before? (photos)

Question

after platysmaplasty to correct bands in front from overdone lipo (both w double board cert celeb ny doc), right side of jaw has bulge that was never there before. left has a slight one but less noticeable. Now 3.5 months post. mass feels very hard. Also hard flat bands appear in location when stretching neck up. What could this be? Can p plasty cause glands to fall when ok before? Doc says just swelling but hasn't changed in 2 months. No lateral pull, only corset in front. No lipo/skin removed.

The deep neck anatomy consists of your submandibular glands, digastric muscles and interdigastric fat. When you address the superficial anatomy (platysma and fat) you can risk accentuating the underlying deep anatomy. You may have always had a prominence that wasn't addressed. Its very difficult to say. My best advice is to wait until all the inflammation is done. Almost a year, well after 6-9 months if possible. Early reoperation although tempting will further confuse the picture. Once it is clear this is no longer scarring and you are a year out it will be evident that the issue is with deep anatomy. Going to a surgeon who can counsel you on your needs and can address the superficial/deep anatomy is key. If I can help answer any questions let me know. Best of luck!

Necks can sometimes take a long time to soften after surgery, especially after a revision surgery and sometimes this can fluctuate for months afterwards. An anterior platysmaplasty shouldn't cause submandibular glands to "fall", as there is nothing attaching the glands to the platysma; what can happen sometimes is that the submandibular glands become more exposed by aggressive resection of the deep neck fat. You should be seen in person and examined to best tell what the firm mass is and to develop an appropriate plan.

Submandibular glands may become more visible after work on the anterior neck, including liposuction and platysma plication.  Whether this is due to removal of fat that had camouflaged the gland or a change in the position and tension of the overlying platysma muscle fibers, it difficult to say.  Swelling should subside, but if the fullness persists and it is the gland, then I suggest that you consult with a plastic surgeon who has experience performing partial resection of the submandibular gland to smooth the neck contour.  Many surgeons shy away from performing this due to it's complexity and risk of complications.
