5 ft 4, 110 lbs, 30H. What size is right for me?

Question

Hi! I'm 30, 5 foot 4, 110 pounds, childless, and a 30H (ow). I'm slender and active, riding horses several times a week (my trainer calls my frame "willowy"). What size would be right for me and my build? I've had one consultation. She said she'd use an anchor cut to get me to a D or C. Does this sound appropriate/realistic?

From your description you will benefit greatly from a breast reduction!  The appropriate size will depend on your goals for surgery.  A C or D cup should look appropriate for you.  The anchor pattern is standard for breast reductions.  Be sure to see a board certified plastic surgeon for your procedure.  I've included a video I hope you find helpful.

There is a fairly wide variability of what can fit your frame. To a large extent, it depends on where you would like to end up. Surgeons can't guarantee a cup size as bra sizing is so variable but you can still shoot for a particular look. C/D sounds appropriate to me.

Thank you for the question. 
Based on what you are describing, it does sound that you have large/heavy breasts causing physical symptoms, and breast reduction may be an option for you. However, it is difficult to tell without seeing the photos and/or in person consultation. 
During breast reduction surgery, excess breast tissue is removed, the breast is lifted and nipple/areola is typically made smaller. It is easiest to think of breast reduction as including the breast lift. 
Schnur Sliding Scale is a scale used by many insurance companies as a guideline for how much breast tissue should be removed in order to improve your physical symptoms. In addition, most insurances in order to approve the surgery, will require for 2-3 letters of medical necessity (from your chiropractor, primary doctor, OBGYN, Dermatologist, etc) demonstrating that your physical symptoms due to large breasts are not improving with conservative management. 
It is difficult to predict the exact cup size, but I typically tell my patients that the goal should be for the breasts to be smaller and lifted, while proportionate with the rest of the body. 
I suggest that you seek an in person consultation with a Board-Certified Plastic Surgeon specializing in Breast Reduction surgeries to further discuss your options, specific techniques, concerns and expectations. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and evaluate their results.

Unfortunately, you have not provided photographs.  However, the technique I recommend is The Bellesoma Method.  This will reduce and reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage.  The weight of the breast is transferred to the underlying pectoralis major muscle resulting in pain relief without excessive reduction.  Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained.

breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small.You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals.
