I don't actually want my natural lip shape lifted or altered -- only excavated so it can be seen LOL. On the right side of my face, some tissue has descended (I'm in my 40s) and is literally burying my lip corner. My skin is in great shape and I'm worried that if a very sizeable triangle of skin were removed, it would leave bad scarring. My questions are -- is it considered a lip lift even if skin is just being removed? Would this procedure be with a scar in my case?

While is good to be concerned after four rhinoplasties likely the skin of the lip is undamaged. The skin right below the nose may be a little bit thinned out, but that skin ultimately will be excised by doing the lip lif.t so a conservative lip lift in this case will probably help and the skin...
Yes, you can still get a very conservative lip lift approximately 4 mm. With 4 mm of excision there will be no elevation of your incisive show. I would also recommend tiny bit of fat grafting as well laterally and medially to give you a little more fuller lip without causing any more incisive show.
