Tubirnectomy and septoplasty due to concha bullosa. (photo)

Question

I have attached a view of a ct scan axial view of my cranial bones that shows the following. I have a concha bullosa. Doctor will perform tubirnectomy and septoplasty. I also have a big nose hump. I haven't spoken to my doctor about the specifics of the surgery and it will be a couple weeks before I see him again, so I am wondering if rhinoplasty may be necessary when doing a tubirnectomy and septoplasty at the same time

A concha bullosa is simply an air pocket located inside the middle turbinate. This can be opened up to prevent headaches and nasal obstruction. A sub mucous resection of the turbinates can reduce them in size  and is called a turbinectomy. A septoplasty is performed for a deviated septum when there is blockage of airflow in the back of the nose do to obstructing cartilage and bone. These procedures are all performed for medical necessity and do not change the shape of the nose. A rhinoplasty is performed for cosmetic purposes to change the shape of the nose. The dorsal hump can be removed which is composed of bone bone and cartilage and then osteotomies are required close the open roof created from the hump removal. Functional and cosmetic nasal surgery can both be performed at the same time, but patient must pay for the cosmetic portion of the surgery. Turbinectomy and septoplasty can usually be performed without a rhinoplasty.  However, if you are seriously considering a rhinoplasty, doing it in conjunction with the septoplasty and turbinectomy is probably the way to go.  You should discuss this with your surgeon.

Removal of a dorsal hump is most often not a "necessary" procedure but rather a choice regarding aesthetics. Rhinoplasty surgery can be a very fulfilling and pleasing addition to functional nasal surgery. The septoplasty and concha bullosa removal portions of the surgery would be considered functional, medically necessary procedures. You should discuss this further with a board certified plastic surgeon.
