I had a lower facelift and neck lift, along with facial fat grafting and a bilateral lower blepharoplasty done 8 months ago. I am very pleased with the face and neck lifts. However, the removal of excess skin under my eyes did not seem to do much. My biggest concern are the scars under my eyes that are still very visible, especially when I wear makeup (which I am not wearing in this photo). The scars are not red, but have not flattened out at all. I return to my dr later this week for my final post op check up. I am hoping there is a procedure that can help with these scars. Laser? Hoping others here can weigh in with advice and options that I can discuss with my Dr. Thank you!
Answer: Possible Treatment Options for Lower Blepharoplasty Scars It's understandable to feel concerned about the appearance of scars following a lower blepharoplasty, especially when they remain prominent after several months. During your upcoming post-operative check-up, there are several treatment options you can discuss with your doctor to improve the appearance of these scars: Laser Therapy: Fractional laser treatments, such as CO2 or Erbium lasers, are often effective in reducing scar visibility by promoting collagen remodeling and skin resurfacing. Microneedling: This procedure can help improve scar texture and appearance by stimulating collagen production in the skin. Steroid Injections: If the scars are raised, intralesional steroid injections can help flatten them. Topical Treatments: Silicone gel or sheets can be used to improve scar texture and reduce their prominence over time. Chemical Peels: Light chemical peels might help in improving the texture and tone of the skin around the scars. Surgical Revision: In some cases, minor surgical adjustments can be made to improve the appearance of scars. Dermal Fillers: If there is any indentation or volume loss around the scar area, fillers might help to smooth out the skin surface. It is essential to have a thorough discussion with your physician about these options to determine the best course of action based on your specific skin type, the nature of the scars, and your overall treatment goals. Your doctor will be able to assess the scars in person and provide a tailored recommendation.

Answer: A resurfacing laser such as CO2 can help improve these scars. Our office has many different treatments for scars depending on the depth of atrophy or hypertrophy. Scars require a combination approach using our HI DEF protocol which uses a combination of lasers, topical scar modulation, and RF treatments to pixelate scars, improve contour, reduce hyperpigmentation, and improve the visibility of the scars so that they are no longer visible in ambient light. Everyone has a unique pattern of scarring and we employ our HIDEF technique to improve texture, color, contour, and topography. We often use a combination of treatments that begin with a careful examination to examine the micro texturing of the acne scars. Atrophic scars and indented scars can benefit from injectable fillers such as Radiesse, Sculptra, and Restylane with or without subcision to remove tethered scars and atrophic depression. Hyper-redness in scars can be improved with V beam laser, Excel V, BBL, IPL, and Venus RF to improve the redness and reduce the visibility of deep scars. Fractional Thulium laser and clear & brilliant can also reduce the inflammation and redness of the scar when combined with topical therapy. Fractional lasers such as Fraxel, erbium, and CO2 laser can be combined with radio frequency and also fractional RF to improve textual issues and microtopography. Subcision and TCA cross are also used for deep ice pick scars and narrow valley scars in order to improve the micro texture. Above all, our combination approach with our clinical nurses and board-certified plastic surgeons can improve the quality of your skin by using at home Melarase creams combined with PRP treatments in our office to improve your skin quality and tone. I recommend getting a formal evaluation with an experienced team of experts in order to reduce your scarring. Finally, hyperpigmentation of scars can be improved with at-home Melarase AM, Melarase PM, and Melapads to help reduce active PIH and melanin deposition. Best, Dr. Karamanoukian Realself100 Surgeon
