A Botox lip lift, performed by an experienced physician injector, can help to diminish the appearance of a long upper lip and philtrum, without affecting the smile and teeth show. Likewise, an appropriate filler, such as Belotero Balance, can be used to enhance the vermilion border (the border between the red and white lips only) without affecting the body of the lip itself. Botox is a great option to help flip the upper lip up. To achieve this effect, Botox is injected just above the vermilion border in about 2-3 areas per side of the upper lip. It will not shorten the philtrum, but with the lip flip effect, it will make it look like the philtrum is shorter. I agree that filler would achieve the opposite effect and pull the philtrum down, especially if it is injected in the wrong place and too much filler is injected. Surgery is another option to shorten the philtrum, a procedure called a lip lift, for which the incision in hidden under the nose. If you have a history of keloid formation after surgery, I would recommend against a surgical option at this point in time.

I think your lip shape and relationship to your nose is beautiful. I wouldn’t touch it. If you really want to shorten your philttum a lift lip will work. I don’t have an opinion regarding Botox tightening.
