I am a size 32a bra and if I workout I am much smaller. I would like bigger breast without implants. I am looking into fat transfer yet I would like to go up to a 35. What is my best option?

Excellent question. 
Yes, you can have a natural breast augmentation with fat. Fat grafting is a great solution for someone who wants to bring their breast size up a cup or so and use natural tissue vs. an implant. The procedure entails using liposuction to remove fatty tissue from unwanted areas of the body such as your abdomen, hips, or flanks, and transferring this into your breasts for natural looking fullness. 
  
Transferred fat is living tissue that grafts onto the existing cells. Most women find that they retain 50-80% of the fat from each fat transfer procedure. The great thing about a fat transfer to the breasts is that once it is injected, it becomes part of your body, meaning if you gain or lose weight, the transferred fat can do the same. Additional procedures can be performed over a 2 to 6 month period until the desired results are achieved. But most fat transfer breast augmentations can increase the breast from half to a full cup size. 
  
The cons are that fat transfer isn't a perfect replacement for breast implants. If a more dramatic increase in volume is desired, I may recommend implants in conjunction with your fat transfer procedure. A fat transfer can easily be combined with an implant or used as a secondary procedure to fill in areas that are flat or thin. It should not affect nipple sensation, mammograms, cancer risk or breast feeding.

In general, 200 cc per breast is typically the upper limit of fat injection recommended by most PSs.  It is not that more cannot be injected; but on average, this has been determined to be the most that can be expected to live during one transfer. Fat transfer to the breast could improve the fullness of the breast, particularly at the top of the breast. If you want more than a one cup size increase, this may require multiple treatments to reach the size you desire as only 50% of the fat typically survives.   In general, the abdomen and thighs are preferred areas for harvesting fat.You want to be happy with the outcome, but choosing an implant based on an aesthetic goal that exceeds your anatomic capacity may lead to complications and possibly revision.

Fat transfer for breast enhancement is best for more upper fullness using smaller volumes. Bra cup size can increase from A to B - the number 32 usually does not increase to 35 (this is the measurement of your chest above the breasts). Usually, 50-60% of the fat stays.For larger than 1 cup size, implants are more reliable.An exam and consultation with a plastic surgeon who performs these procedures is recommended to confirm you are a candidate as well as discuss your options and expectations.

Fat grafting to the breast can increase the size by about one cup. So, in your situation you may realistically be able to go from a 32a to a 32b with one procedure. If you wanted to go larger then it may require additional fat grafting procedures at a later date. The number 32 is a measurement of circumference of your chest. Fat grafting to the breast will not increase you from a 32 to a 35.

Thank you for posting your question. Breast augmentation with fat transfer or implants will alter the size of your breasts but not your chest circumference underneath your breast which is what the 32 is referring to. It will still be 32 afterwards but your cup size may increase from A.
Please be aware that while fat transfer can give a very natural and long-lasting breast augmentation, it is really only practical for small to modest size increases e.g. one cup size. It often needs more than one surgery as only so much can be transferred in one sitting to the breasts and your body will absorb some of it during the healing process. What remains after that is permanent. If you are hoping for a bigger increase in size, then breast implants are a better option.

Fat transfer will probably not increase you to size 35, nor does it increase the size of your ribcage. At best, you can expect up to one cup increase in size with a transfer. I would suggest meeting with a board certified plastic surgeon and discussing all your options.

Thanks for the question.Fat transfer to the breasts works very well, but not nearly compared to the buttocks.  You are able to increase the breast size by one to one and a half cups AT MOST in a single procedure.  The '32' is the measurement of your chest.  This remains constant.  The 'cup' size changes with the addition of volume.GO for several consultations and do your homework.Best of luck with your procedure.Dr T
