Major splits after a PDM. Dr wants to do a correction at 6 weeks PO. Better to do closure with sutures or skin graft?

3 weeks PO. breast split majorly. Dr wants to close up breast in 3 wks . He said 2 options. 1)Close up breast with sutures. Risk split again. No guarantee wont split again. 2. Skin graft (from thigh) to open area on breast. Will definitely close up wounds but will leave breast uneven & blochy. Another Cosmetic surgery later. I would like option 1 but am scared to death that I will split AGAIN!

There are many ways of handling this. To quote the great Dr. Russell, "when there are many ways to do something, no one way is best." There are different types of wound care that can be applied to heal these wounds over. They would take quite a while. Skin grafting is a very good option. I wouldn't recommend closure. Sometimes you need to use more than one method to heal a wound.I realize that this doesn't give you the one answer you wanted. In your case, you just keep working on it until you solve the problem. I would also meet with a nutritionist. I've found that to be helpful in some of my wound patients.

I would follow your surgeons instructions. There is a wound vac technology which could prove valuable in shrinking the wound fairly quickly. Insurance may cover it. Just a suggestion.All the best,Talmage Raine MD
