A softer, more feminine lower face is generally characterized by a tapered, V-shaped jawline, a well-proportioned chin and appropriate projection, smooth transitions between the cheeks and jaw, and less prominence of the jaw angles. One of the most effect ways to create the illusion of a slimmer jawline is to increase the chin projection. A more projected chin elongates the lower face and creates a more tapered appearance. The least invasive approach is hyaluronic acid filler. The filler is placed directly on top of the chin bone to increase projection. Approximately 2-3 cc's are often required to produce a noticeable change. The advantage of filler is that it is performed in the office, requires little downtime, and is reversible with hyaluronidase if desired. The tradeoff is that filler gradually dissolves over time and in some patients, can migrate. Two surgical methods that increase chin projection include a sliding genioplasty and a chin implant. A slide genioplasty involves making a controlled cut in the chin bone and moving it forward. The bone is then secured with titanium plates and screws. This procedure is more invasive and considerably more difficult to reverse. In some patients, a small palpable step-off can be felt where the bone was advanced, although this is generally not visible externally. My preferred surgical option is a chin implant. The implant is placed directly on top of the chin bone through either a hidden incision inside the mouth or a small incision beneath the chin. Chin implants do not require cutting the bone and can be removed or exchanged relatively easily if desired. For patients with prominent jawbones, mandibular contouring surgery can permanently reduce the height, width and angularity of the lower face. Through incisions hidden inside the mouth, the jawbone can be reshaped and the chin sculpted into a more oval rather than a square appearance. In carefully selected patients, buccal fat pad reduction can be combined with jawline contouring to improve definition beneath the cheekbones. This creates the illusion of higher cheekbones and further emphasizes a slimmer lower face. Submental liposuction combined with energy-based devices, such as FaceTite and Renuvion can complement these procedures by tightening the soft tissue while also producing a degree of fat reduction (lipolysis). These treatment are generally best suited for younger patient with good skin elasticity and mild to moderate excess fatty tissue who are looking for a modest but meaningful improvement in jawline definition without undergoing a traditional surgical lift. For patients with more advanced skin and soft tissue laxity, particularly those with jowling and loss of jawline definition that creates a broader, squarer lower face, a facelift is often the most effective option. Ultimately, the ideal treatment depends on what is contributing most to the appearance of a broader lower face. Whether that is chin position, jawbone shape, enlarged masseter muscles, excess fat, or age-related soft tissue descent. Sometimes, combining several complementary techniques rather than relying on a single procedure is best. Hope this helps!
