After graduating from De La Salle Dr. Tamplen attend UC Davis on a full tuition scholarship. He graduated early, ranking #1 in the College of Biological Sciences with a 4.0 GPA and was awarded The College Medal: the highest honor offered by the University to 1 graduating senior per year. Dr. Tamplen was recruited to UCLA Medical School with a Full Tuition Merit Scholarship and at UCLA achieved one of the highest scores in the nation on the medical licensing exam and was elected to the Alpha Omega Alpha Honors society. He then attended top ranked, five-year surgical training program in Head and Neck Surgery in his hometown of San Francisco at the UCSF. After residency Dr. Tamplen completed a prestigious Fellowship in Facial Plastic and Reconstructive Surgery with the former president of The Academy of Facial Plastic Surgery, Dr. Edward H. Farrior. He was awarded the Jack R. Anderson Award for Scholastic Excellence in 2018. This prize is awarded annually to the U.S. or Canadian surgeon who achieves the highest combined score on the written and oral portions of the American Board of Facial Plastic and Reconstructive Surgery examination. This is the most prestigious award given by the ABFPRS to only one graduating fellow per year. In addition to my aesthetic work, Philanthropy is a pillar of my practice. I am committed to helping children throughout the world with limited medical and financial resources receive surgeries, including cleft lip and palate reconstruction. Q: Will Rhinoplasty fix septoplasty problems?

A: Hi,
I am so sorry you are struggling with these problems after your first surgery. When nasal passages constantly feel dry and bleed, often there is still some form of obstruction on the inside or sometimes a hole in the septum called a septal perforation. I would recommend getting evaluated by a facial plastic surgeon to see if you have any of these issues. A rhinoplasty can treat both and can actually open the size of the nasal airways even if the outside result is the nose looking smaller. I do this all the time for my patients. Definitely keep using the nasal saline as well. Hope that helps.

A: Hi,
I am very sorry you are not happy with the outcome of your surgery. I can see exactly what you are talking about in the photo. The bump is bone and or cartilage and it is not swelling. If you were originally happy with he outcome and now there looks like more of a hump it is because the tip of the nose has dropped. This is very common after endonasal or "closed" rhinoplasty. Endonasal rhinoplasty focuses primary on removing the dorsal hump and not on supporting the nasal tip frequently leading to this issue. I have found open rhinoplasty with an emphasis on tip support is the best way to get consistent excellent outcomes. I would recommend an open revision rhinoplasty for you to support your nasal tip and provide you with the ideal dorsal profile. I would recommend seeing a board certified Facial Plastic Surgeon with extensive experience in open and revision rhinoplasty for consult.

A: Thank you for the question and the photo. I am sorry you are having issues with your incision. It looks like you had what we call an Alar Base Reduction as a part of your rhinoplasty. This is done to make the bottom part of your nose narrower and requires incisions like those on the side of the nose. The incisions usually heal very will but sometimes with the skin is sebaceous or has a lot of oil glands, the wound edges can pull away a little causing the appearance in the photo. This can happen but is easy to fix. At this point I would recommend having a small scar revision where the area is recut and sewn back together in effort to improve the scar. This can be done in just a few minutes in the office. Hope that helps.

A: Hi,
First congratulations! This is actually a question that comes up a lot. Some of my happiest patients got their rhinoplasty less than a year before their wedding and were so happy they did. It gave them more confidence and they loved their wedding photos. To be safe I would give yourself 6 months but that should be plenty of time to achieve a nice result prior to your wedding. Looking at your photo I don't see anything challenging about your nose and you have nice skin quality. I would see a board certified Facial Plastic Surgeon for consult and go for it. Q: I hate my side profile, what can I do to fix it? (Photo)

A: Hi,
First, I don't think you have anything to be self conscious about at all. No one is born with the perfect profile and you have many great features. However, I do see a couple areas that can addressed to remove distraction and improve your overall appearance on profile. The nose and chin are out of balance making the nose look larger than it actually is. This is partially caused but some reduced chin projection and also by the nose being a bit short and over projected. This also leads to a long white upper lip reducing the size of the red lip. I would suggest a rhinoplasty in combination with chin augmentation first to balance the face, bring the nose down slightly for a more elegant appearance. This will help balance your profile and reduce the size of the thin part of the upper lip. We could consider lip augmentation several months later. I think you would be a great candidate for a consultation with a board certified Facial Plastic Surgeon.
