Restylane
Q I had Restylane under my eyes in December and now, in May, they are still swollen and have a light blue tint. Advice? (photos)
A

Based on your description, the blue tint and swelling is likely due to the filler being placed too superficially under the skin.  The bluish hue is referred to the Tyndall effect, which occurs due to how different wavelengths of light scatter depending on the substance they encounter.  Blue light scatters about 10 times more than red light when passing through very small particles.  For this reason, the hyaluronic acid (HA) filler beneath the skin scatters more light of shorter wavelength, resulting in a bluish discoloration.  The swelling you're experiencing can partly be due to the fact that the filler has been placed more superficially under the skin, as well as due to the fact that HA fillers are hydrophilic (attract water), which can result in more swelling, especially if there was a large volume injected.  As the skin under the eyes is very thin (about the thickness of an eggshell), there is typically a higher risk for the Tyndall effect and swelling in this area post-filler.  Occasionally, firm massaging can help improve the aesthetic result, but given that this developed months after your filler, this will unlikely solve the problem.  The mainstay treatment is typically to dissolve the HA using hyaluronidase, which often results in resolution within 24 hours.  However a second treatment with hyaluronidase may be required.
