Dr. Adam Schaffner, is a triple board-certified, double fellowship-trained plastic surgeon practicing in New York City. Dr. Schaffner honed his skills while training in a breast and aesthetic plastic surgery fellowship at Georgetown University Hospital in Washington DC and an advanced facial plastic and reconstructive surgery fellowship, during which he was a member of the medical staff at Stanford Hospital and Clinics. Dr. Schaffner is a diplomat of (certified by) the American Board of Plastic Surgery, the American Board of Facial Plastic and Reconstructive Surgery, and the American Board of Otolaryngology – Head and Neck Surgery. My experience with Dr. Schaffner was great, I had two surgeries with him, including a fat transfer from thighs to butt and a breast augmentation a while back. Dr. Schaffner does great work, he’s reliable and trustworthy as well.

Q: Is it possible to get cleavage after your first surgery?

A: Breast implant size selection is important and based on many factors. The breast implant has three dimensions: height, width and projection. These must be taken into account to help determine a range of sizes which may be best after being examined by a board certified plastic surgeon. Computer imaging (e.g., VECTRA) may help you visualize the simulated results with different breast implants.

A: It appears that you have experienced bottoming out of your breasts following your breast reduction and weight loss.  The appearance of your breasts and chest may be improved with a breast lift.  Reinforcement with an acellular dermal matrix or mesh may be advised to minimize the risk of recurrence of bottoming out.  If you desire more fullness than you currently have with an unpadded bra and/or more camouflage of the sternum, you may consider adding volume using a breast implant and/or fat grafting.

A: While cost is an important question to ask, it is even more important to make sure your doctor is a board certified plastic surgeon who specializes in breast augmentation surgery.  Look at before and after photos of patients on whom the doctor has performed breast augmentation and speak with patients who have had a breast augmentation performed by that doctor.  Once you choose a doctor with whom you feel comfortable and whose results you like, I suggest using computer imaging (e.g., VECTRA) to assist with size selection.  After you have done this, then I recommend focusing on costs.  Be sure to accurately compare price estimates asking about all fees (e.g., surgeon, implants, garments, facility/operating room, recovery, anesthesia).

Q: When is it safe to have facial fillers after Septoplasty?

A: While there is no definite answer, I suggest waiting until all bruising and swelling have resolved and your surgeon has cleared you to have fillers.

A: To properly answer your question, physical examination by a board certified plastic surgeon or facial plastic surgeon is recommended.  Following examination and review of the operative reports, further recommendations may be made to discuss how to best fix the "telephone effect" you do not like.
