A Hi thanks for asking this question. First of all we need to make sure you are measuring your penis correctly. The standard way of measuring is to stretch the penis as far as it will go and measure from the base to the tip of the head. The average stretched length is about 5 1/4 inches. To meet the medical definition of a "micro penis" the stretched length would need to be less than 3 inches. If you are measuring correctly your measurements definitely do not classify you as having a micro penis and fall within the normal range. Dermal fillers do not increase the erect length of the penis, they can only help increase the girth and make the flacid penis look longer. There are some risks to penile fillers including lumps, nodules and decreased sensitivity and if not performed correctly, it could lead to permanent disfigurement and erectile dysfunction. This is not a procedure I would recommend for someone at your age. I usually turn away patients who are so young and do not have an actual micro penis. However, If you feel you do need to do something and want to get a treatment that is less risky and more natural there is now a treatment protocol available called "P-Long" this treatment involves using a vacuum pump and a stretching device along with injections of platelet rich plasma. This protocol does involve a daily routine that you would need to do and could be a significant financial investment for a young person, but the cost is less than having filler injections. (I absolutely would not recommend suspensory ligament ligation or implant surgery in case you were thinking of those procedures). Go with the P-Long or just be reassured that your measurements are quite normal and you don't need to do anything.

A This is a great question. I have performed about thousand fat transfers over my career and I have not seen biofilm form around fat. That is not to say it couldn't form around fat, just that it is unlikely. Then again, I have preformed several thousand filler injections and have only seen 2 cases of biofilm. The reason biofilm forms around a foreign object is because your immune system cannot get to it. You need a blood supply to the area for your immune system to fight the community of bacteria that find sanctuary on the foreign object. Because a fat transfer implants living tissue and the implanted cells get their own blood supply, an infection that forms on the fat cells can be fought off by your immune system. Although hyaluronic acid is a substance that is normally found in our bodies, a bolus of HA injected into you does not get its own blood supply so bacteria can hide in there. An improperly performed fat transfer can form an infection because if you put too much fat in, the center of the implanted fat will not get a blood supply and bacteria can fester in there causing an abscess. Since you have had biofilm form and it is difficult to completely eradicate, I would not recommend any more fillers and even a fat transfer could be risky. You might want to consider a PRP treatment or a PRP gel treatment like AutoCorre. PRP has antibacterial qualities and sends a signal to stimulate creation of new collagen and blood supply. The gel dissolves away so you don't have the risk of a biofilm forming on it. Also, make sure you go to an injector who is very diligent about sterile technique.
