Q: Is a deep plane facelift a high SMAS face lift where the ligaments are released?

A: You are asking a technical question which is commonly discussed at conferences among facelift experts. 

In essence, when the surgeon dissects beneath the SMAS layer s/he is able to release ligaments which in other facelift techniques limit the confluent repositioning of the facial tissues. 

Both, the deep plane and the high SMAS face lift procedures do this and are allow the surgeon to more effectively improve the overall facial contour. 

A high SMAS lift requires more surgical dissection - but this is a minor difference. 

Additional advantage of the deep plane facelift is that the skin maintains a robust blood supply leading to decrease in complication. 

Q: Will Botox or filler stop my top lip curling inwards when I talk & help uneven lips? (Photos)

A: Great question! You say your lips are uneven, though in the photo you sent, your lips look beautiful and quite symmetric at rest.  A small amount of filler can correct any asymmetries you see.  What I would recommend trying first is a small amount of botox/dysport to the top lip, which can help with the "curling in" if done correctly.  It's a quick, cheaper and less invasive option.  Some people do find they have some initial difficulty drinking through a straw, or report that their smile looks and feels different.  The good news is that these changes are self-limiting, as neurotoxins only last 3-4 months, and will start to improve as you regain some strength in the orbicularis oris muscle.  It is definitely worth a shot to see if this corrects your concern!

Q: I'm 24 with naturally deep smile lines. Is there a way to fix this? (photos)

A: Dermal fillers can be used to treat this area and often will result in a softening of deep lines. Hyaluronic acid fillers are often used in this area (Juvederm or Restylane), although there are other products on the market which may have more longevity (ie.Bellafill) and thus will not require as much maintenance. I would recommend scheduling a consultation with a Plastic surgeon to perform a full facial assessment and develop a treatment plan to help you achieve your goals.

Q: What are the signs of necrosis after lip fillers? Would my practitioner be able to tell straight away, if they're experienced?

A: Necrosis is a rare complication of lip fillers and choosing an injector who is aware of the signs and symptoms of intravascular occlusion is very important. It is also crucial to have an injector who is knowledgeable in regards to facial anatomy as well as safety mechanisms to prevent these risks from occurring. This risk can often be identified by your provider immediately as signs of blanching of the skin (white color) occur. Other symptoms include pain, skin discoloration and finally necrosis if left untreated. If needed, the filler can be dissolved with an enzyme known as hyaluronidase which works immediately.

Q: Would Botox alone fix my forehead and smile lines, and indents near my eyes and nose? Or do I need filler as well? (photos)

A: Botox will most definitely soften the horizontal lines on your forehead, and can also be used to slim the face and decrease bruxism (teeth grinding) by injection into the masseter muscles. The slight indentations in your central cheek area would be best addressed with hyaluronic acid dermal filler. This same product can also be used to soften the smile lines. A combination treatment of both products would give a great improvement and the refreshed look you are seeking. I would recommend an evaluation with a plastic surgery office for further assessment and development of a treatment plan.
