A: Most of our gynecomastia cases are done under a "lighter" form of general anesthesia using an LMA which means the patient is breathing the entire time on their own although completely asleep. The anesthetic wears off quickly leaving patients feeling better that they might have using older forms of general anesthesia. We feel it is important and safer to work with board certified anesthesiologists in the ORs where we work to give the patient the safest and best experience. A: The most common cause of pain 9 years after breast augmentation is usually capsular contracture or scar tissue forming around the implant. This can certainly be on one side only. This can be determined on an exam by a Plastic Surgeon and would require surgical capsulotomy or capsulectomy to relieve.. A rib related source of the pain is possible ( costochondritis that you mentioned) , especially if there does not appear to be any capsular contracture on exam.. In that case , a MRI might be beneficial if no other source of the pain can be determined..Dr. Price
A: Generally all implants for breast augmentation should be behind the pectoralis muscle ( actually the implants are behind muscle about 75%)_. It was clearly shown many years ago that the under muscle placement leads to less capsular contracture and gives better coverage of the implant especially in the upper part of the breast which is potentially the most visible in a low cut dress or bathing suit.
A: It looks from the photos and information that a 275 cc implant would be fine. It is possible that a "high profile" implant might look better but only an exam can say for sure. Certainly a slightly smaller implant would also work if you are nervous about being too big since you are a small person. Definitely "under the muscle" as you otherwise will have little coverage over the implant. So the breast augmentation should be very good for you. I cant see that you will get much out of liposuction as you are so small. Perhaps a small amount of liposcution or skin removal in the lower abdomen if there is some skin laxity. I dont think you will get much out of fat grafting to the buttock ( BBL) as you will not have much fat to use.. Dr. Price
A: You look like a good candidate for a panniculectomy which involves excising the lower abdominal roll and excess skin and fat. There may be some liposuction done as well. Usually with a panniculectomy there might not be a correction of any diastasis recti ( lax abdominal wall muscle layer) or as much liposuction as the primary goal is to get rid of the excess roll of skin and fat. This differs from a tradition abdominoplasty where there frequently is a repair of the muscle layer and possibly more liposuction. Either procedure can be tailored to the patient needs and carry a high level of patient satisfaction. Dr. Price
