Q: Why don't more surgeons offer scarless arm lifts?

A: In my experience, the mini arm lift via the axilla (arm pit) is only applicable to those with minimal skin excess. It sounds great but it is not possible to lift an entire arm length of skin at the axilla with good results. Most arm skin excess is around the arm and needs to be reduced vertically down the inner arm, especially after massive weight loss. Brachiaplasty +/- liposuction works best. The arm pit scar can pull apart, heal poorly, etc. There is no scarless surgery- there will always be a scar. In my experience, many smaller arm patients can be treated with Vaser ultrasound liposuction to remove stubborn fat and tighten skin without removing skin. An exam and consultation with a plastic surgeon is recommended to discuss your options and expectations.

Q: Lidocaine in tumescent lipo - curious about the safety

A: Tumescent liposuction is very safe. It allows for larger fluid volumes with very dilute lidocaine. In my experience, we place less lidocaine in larger volume cases ans we add more and keep the total amount suctioned to a safe level- generally less than 5000cc. (Problems can arise if the proper formula/ ratios are not followed and patients are not monitored closely during and after liposuction). Further discussion with your plastic surgeon is recommended.

Q: Can a different plastic surgeon do the re-implant secondary surgery?

A: Yes, in my experience, it is very common for a different plastic surgeon to remove, replace breast implants. An exam and consultation with a plastic surgeon is recommended to further discuss your options and expectations.

A: Cleavage is determined by your anatomy- space between breasts, footprints of breasts, width, where breasts end, etc which cannot be changed with larger implants. Cleavage will be different for different body types, breast anatomy which has limitations. The pockets may be opened slightly toward the middle to accommodate larger implants, but breasts will still not touch. May not be worth the risks. Further discussion with your/a plastic surgeon is recommended.

A: That’s a plastic surgeon’s preference and depends upon each patient. If mainly lose skin and not much fat can be treated with skin excision only. In my experience, if there is extra skin and fat around the arm, then liposuction allows me to remove more skin and get a thinner, tapered arm in one surgery. (Liposuction of arm thickness area not removed with skin excision). Most patients do not like the option of liposuction first, followed by skin excision months later. Further discussions/ consults are recommended to feel comfortable with plan.
