Excellent service - the actual procedure was safe and smooth

Dr. Omar Tillo is an excellent surgeon. He was very professional from the video consultation to the in-person consultation all the way through to the procedure and the post-op follow ups. He took great care of me and checked in regularly to see how I was doing. He answered any questions I had in the consultations and then broke down what he is able to do to meet my expectations. The actual...

Q: Is J-Plasma necessary to get a good result? (Photo)
Answered by Dr. Del VecchioJanuary 11, 2024

A: There is no compelling evidence that J plasma tightens skin to any clinically significant degree. The risks of overheating, scarring and skin damage outweighs the benefits.

Q: Considering breast implant removal with a lift and fat transfer to replace the implants? (Photo)
Answered by Dr. Del VecchioJanuary 7, 2024

A: Thank you for your question. The best course of action in your case would be to do an SIEF operation. You could have the implants removed and Fat replaced. Once everything settled down, you could consider adjusting the position of the nipples. If you take the capsule out at the time of the implant removal, it is not possible to safely fat graft, because the fat will migrate into the pocket and you will have fat necrosis. in other words, when a SIEF is done, the capsule must be left intact. Hope this helps!

Q: Can I decide where to place fat when undergoing a breast fat transfer?
Answered by Dr. Del VecchioJanuary 6, 2024

A: Fat grafting to the breast is a great operation when performing the right patient. That grafting does not lift the breast however, so if you’re looking for a breast lift back, grafting is not going to accomplish that. You should probably have an in person appointment with a plastic surgeon in your area who is familiar with fat graft into the breast and you will get the best possible advice and how to proceed. Best regards.

Q: Can I achieve my desired BBL results with this body type? (photos)
Answered by Dr. Del VecchioJanuary 4, 2024

A: You are a great candidate for 360 liposuction and BBL. You could have fat removed from your abdomen waist and back and place some of it it in your butt to make your hips wider and give your butt more projection and cuff. Is this what you're looking for?

Q: Explant and wait or having fat transfer simultaneously?
Answered by Dr. Del VecchioJanuary 4, 2024

A: A technique of implant exchange is reported using recipient-site preexpansion followed by autologous fat transplantation to the breast in patients with breast implants who desired implant removal. Recip- ient-site preexpansion, used 2 weeks before fat grafting, may have both practical and theoretical benefits in increasing the breast subcutaneous space and stimulating the recipient-site microcellular environment overlying the prosthetic implant, allowing the subcutaneous insertion of a sufficient core volume of donor graft at the time of prosthetic explantation. In the cases described, the postexplantation breast volume at 9 months to 1 year postoperatively by quantitative three-dimensional imaging was equal to or greater than the preexplantation composite volume of breast and implant. Preexpansion before implant exchange with fat affords a more abundant space, completely independent from the subglandular or submuscular planes. In this new space, the “third space” of the breast, it is possible to technically place graft into the breast subcutaneous tissue and alleviate breast asymmetry resulting from pocket distortions caused by capsular contracture or by implant pocket drift. Observing breast augmen tation with implants and with fat grafting in the same patient affords a unique opportunity to analyze some of the key differences between the two techniques. Recipient-site preexpansion and simultaneous implant exchange with fat (SIEF) should be added to the list of applications where fat grafting to the breasts may have early clinical utility and portends the use of fat used in conjunction with breast implants to achieve better patient outcomes.
