A: You should be returned to your previous cup size with implant removal so the fat grafting will help get a little volume most likely and get a better shape to your breast. The nice thing about fat grafting is that it can be repeated later on to add even more volume if you are initially disappointed with your breast size after implant removal.
The other thing to consider is wrapping your implants in a mesh or strattice which can almost bring your chances of recurrent capsular contracture to zero.
A: The photos you show are patients with pretty big implants with a more natural look. I think you will certainly have a more natural look because of the natural shape you have now and in order to get that look you will need a large implant at least of 450 ccs.
A: The general rule is if you want your breasts to be larger than an increase of at least 100cc is recommended as changes of 50cc or really less than 100 cc is hard to notice a difference and for patients that want to be significantly larger than over 100 cc increase in implant size is suggested.
A: Based on your photos I don't think you would need a lift and you should be able to achieve a good look to your breasts with an augmentation only.
A: Your best option hands down is a lift with the implant under the muscle. You need a lift and just placing an implant would not give you a good appearance to your breast. You don't have enough tissue to go under your glad, but if you did put big implants in under the gland you would just have big droopy boobs. If you need a lift you need a lift because contrary to most people's understanding breast implants don't lift.
