	This woman had a long history of severe, progressive temporomandibular joint degeneration. The joint degeneration caused an open bite malocclusion where the only teeth that touched were her molar teeth. The upper jaw grew too long, and tilted down on the right, causing too much gum tissue to show when she smiled. Her lower jaw rotated backward making her chin appear weak, and creating a large gap between her upper and lower lips. After orthodontic treatment, upper jaw orthognathic surgery, and total reconstruction of both temporomandibular joints, her bite was normal and beautiful.

This patient had a skeletal reason for his teeth being out of position and rather than trying to fight nature, Dr. TImothy Osborn decided to fix both the upper and lower jaws with reconstructive jaw surgery, septoplasty, and limited access rhinoplasty. This patients aesthetic concerns were the crowded teeth, showing too much gum when he smiled, the lack of his chin prominence, and an unsupported nasal tip. To correct these, we moved the upper jaw forward and up, moved the lower jaw forward, and performed a Septoplasty, and Rhinoplasty from the underside of the nose It was a team effort, in coordination with Dr. Ella Osborn who moved his teeth to allow for a normal bite and perfect smile.Now he is a different person; not once in the 6 months leading up to his surgery did I see him smile, and at his final follow-up appointment, you couldnt get him to stop.

Female patient presenting with a common case of orthodontic camouflage treatment and its subsequent failure. A maxillomandibular advancement (MMA) procedure, otherwise known as double jaw surgery, was performed. The patient underwent a LeFort 1 osteotomy on her maxilla and a segmental bilateral sagittal split osteotomy (BSSO) on her mandible along with a counter-clockwise (CCW) rotation to bring the occlusal plane into it's optimal position and optimize both form and function. The lower jaw (mandible) was ultimately brought forward by 12.94mm at the pogonion (the forward-most point on the chin), and the upper jaw (maxilla) was brought forward by 5mm. A counter-clockwise rotation of approximately 8 degrees was also performed to lengthen the lower third of the face to balance the bimaxillary movements, further improve the airway, stabilize the occlusion (the bite), and achieve complete facial harmony. 3D-printed custom orthognathic hardware by Stryker was used for the procedure.

Especially with a genioplasty, her jawline is now more defined and sculpted! Her overall appearance is tightened and lengthened, creating facial harmony and balance.
