With huge lateral flap on the left side, coming into the outside left labum like a "river delta," this patient's surgery had to be designed to accommodate the significant anatomic differences of the 2 sides. We did a modified V-Y procedure on the left while just bringing up the right "V" to "smooth out" the hood on the right. We also modified the bottom & did a minor perineoplasty.
