Great question.  You have to start asking yourself questions like - what do I want to look like when I'm done?  Liposuction often sounds more attractive, as its usually less expensive, and the incisions are much more inconspicuous.  The problem is that when you already have some loose skin on your belly, and then you remove volume, you'll likely have MORE loose skin after liposuction - this is likely not the look you're going for.  A tummy tuck will be able to contour the abdomen, remove the loose skin in addition to removing some of the fat, and give an overall nicer shape.  Yes there is an incision, so it's important to find a surgeon who is skilled in making scars as thin as possible, but my bet is you'll be happier with the result of the tummy tuck more than with the liposuction alone.  In my practice, I usually include liposuction of the flanks as a normal part of a tummy tuck.  Lipoabdominoplasty combines the best part of both procedures, giving a thinner look all around.

In years past doctors focused on adding volume directly to the area around the mouth to smooth out the nasolabial folds.  If you look at how people age, they actually tend to lose volume in the cheeks and tear troughs, and because of this, the skin and tissue in these areas drop and weigh down on the nasolabial folds.  Restoring volume to the cheeks, tear troughs, and even the areas in front of the ears and right below the cheeks can give you that feminine look you're going for.  Adding all that volume tends to get expensive with fillers.  In my practice, fat grafting to these areas tends to give a longer lasting result for less money than all of the filler it would have required.

Assuming you're a good surgical candidate, liposuction of the jawline/neck alone would give you a good result, but adding Renuvion treatment to the area would tighten the skin in addition to thinning out the fat.  This is something that may be done in an office setting under local anesthesia.  I would start with those.  Renuvion takes a little time to see the full tightening effect, but it is well worth the small extra expense.

It looks like you're heading in the right direction.  Rhinoplasty will be able to help the central portion of your midface, but you're right to address the cheeks also.  It looks like you may have lost some volume in the tear troughs, which has the tendency to make your face look longer than it used to.  Some of the Restylane/Juvederm products would be able to address this, but fat grafting would too.  It may be worthwhile discussing this with your surgeon, as either could be placed at the same time as the rhinoplasty.

The ideal clearance letter comes from a doctor who knows your history well.  Going to someone who has never seen you before may be better than having no clearance letter at all, but having the input of someone who is well aware of the problems you face is much more valuable in the long run.

While you may feel ok accepting the risk of having multiple larger procedures done while you still smoke, your surgeon likely does not.  They have your welfare in mind - they want you to heal well, and they want you to look your best.  If you're going to spend the money, you likely want the best possible outcome - this comes with stopping smoking for a month before and a month after surgery.

Costs not only vary by region and experience of the surgeon, they also vary by demand - some surgeons may be busier than others.  This is not necessarily to say that the most experienced surgeon will be the highest priced.  If you are concerned, you could always consult with other surgeons and see what seems most appropriate, and who is best fitting your needs.

The lower eyelids tend to have many blood vessels that are subject to damage during injection.  Although discouraging, bruising after injection is common.  When bruising does occur, it can take up to 14 days to fade.  Sleeping with your head of bed elevated, using ice packs or cold compresses, and possible treatment with arnica can improve the appearance.  If the bruise doesn't look like it's improving by 10-14 days out, checking in with your physician is wise.  Together, you can find a solution that works best for you.

At this point your scars are healed, albeit in a fashion that is not as cosmetically appealing as prior to surgery.  Any further procedures should be discussed with your plastic surgeon so that you are aware of potential benefits and risks.  Your plastic surgeon should also be able to give you a realistic idea of what to expect in terms of outcome.  In your case, scar revision may not be as difficult or complicated as you may think.

While bruising may be a cause for alarm, the amount of bruising you have is commonly seen after breast augmentation.  If you give it another week or so, the bruising should subside.  Usually bruising presents at the same time as postoperative swelling.  As the one goes away, so should the other.

Good experience so far - 5'8, 134lbs, 32B to 32D/DD

Sharing my experience because these were super helpful when I was looking for a surgeon. I'm 5'8 and 134lbs. I was previously a 32B, I'm guessing I'll be around a 32D/DD when sized. I ended up with 345cc in my left and 365cc in my right. My rib cage is slightly uneven so we used different sizes to help fix some previous asymmetry. I had over the muscle subfascial placement. Both implants were high profile, soft touch (AKA cohesive silicone) natrelle. I'm only a week post op but I feel almost back to normal, aside from the incisions occasionally hurting when I first stand up in the morning. I think Dr. Nimtz did a great job. I chose him over several doctors that I consulted with in Louisville because he offered subfascial placement. His wife (she's the office manager I think) was super friendly and kind and gave me her phone number in case I have any questions or issues. On surgery day I had a great experience. They have an on site surgical center. I never had anesthesia before so I was very worried. The anesthetist was super nice and explained everything very well. Surgery day happened so quickly that before I knew it I was awake and the surgery was over. The pain after surgery lasted only a couple of days, and I regained full range of motion in my arms quickly. I was prescribed a nausea patch that I put on before surgery, antibiotics for after surgery, and both painkillers and nausea medicine but I barely needed those. I am also taking singulair for capsular contracture prevention. I am attaching some pictures, even though I am only 1 week post op I am super happy with how they look so far; I know they will continue to settle and look even better. I didn't care about looking super natural, I wanted a round, slightly augmented look so that's why I chose above the muscle despite not having much tissue to start with. I was thankful that the surgeon let me make that choice; some surgeons I previously went to would only do over/under without letting me choose. I had some previous asymmetry before surgery where my left breast fold was slightly higher than my right so I am waiting for the left implant to drop slightly to be even with the right side. I'll keep this updated as I heal.

Less than 2 weeks until my breast augmentation! I’m a 45 year old mom of 4, 5’3, 120lbs. Going with Ideal Implant, around 450cc. Currently 34a, hope to be about a D cup. Nervous but excited! I can’t wait to see how they turn out, so tired of the itty bitty titty club ;) Updated on 29 Oct 2022: My surgery was moved to October 4th and every went great! So happy with my decision so far. I ended up with 405cc/left and 425cc/right. Dr wasn’t able to go up to 450 like I’d hoped, I really hope that doesn’t end up bothering me too much. Updated on 20 Dec 2022: Weekly update photos! It’s hard to see any difference from day to day, but with weekly photos you can see the subtle changes. Feeling completely back to normal and they’re getting softer and relaxing more. Updated on 14 Jan 2023: I bought a few new bras since I’m 3 months post surgery! I’m wearing 34d in every brand I’ve tried so far. I’m really happy with the results, I just wish they were about twice as large. They feel really small to me! For anyone concerned about the weight of the implants, believe me when I say you can’t even tell they are there! I run 4 miles, 3 nights a week and 10 miles on Sundays and it’s like nothing has changed! When I tried on the sizers in my surgeons office they felt really heavy sitting on my chest. I wish someone had told me that isn’t how it feels when they’re inside you, I DEFINITELY would’ve gone bigger!!! I went with under the muscle so I assume that’s the reason for the weight not being an issue?

50 y/o Stargazing Nipples Corrected

I had a small breast reduction (more like a lift) in 2003 by another plastic surgeon, which resulted in stargazing nipples. Lowering the nipples is difficult to correct. Dr. Nimtz was able to lower them into a normal position. He performed ‘breast lift incisions at IMF’ in 2019 and nipple reshaping in 2020. I am so happy. I never thought my breasts would look so natural. Updated on 5 May 2021: Wanted to add a photo of how my scars look after Dr. Nimtz performed breast lift at IMF 8/2019. Little redness remaining at cleavage. Used paper tape for first few months + silicone scar sheet strips for majority of time post-op.

Couldn't Be Happier! This is the Guy to Go To!

My results are so natural looking. People are telling me "gosh you just look so good" not knowing why. I still look like myself, but a much BETTER version. I did consultations with other doctors before Dr. Nimtz and I was not confident with their understanding of what I wanted. It was more about what these other doctors wanted - and they wanted to sell me more procedures! They would do things like say you've also got problems here and here and here, trying to make me feel insecure so I'd get more work done. Dr. Nimtz was not like that at all. During our consultation his emphasis on natural looking results made me feel very comfortable with his ability and perspective. And he delivered exactly what he promised. I look better and fresher and not a hint of being synthetic looking or "over done". Be sure to get a consultation with him before you do facial work.

Highly recommend Dr. Jared Nimtz Updated on 7 Nov 2018: Tummy tuck incisions are healing well, however the breast lift is not. Dr said it is normal and it’s just the dissolvable sutures dissolving,but I’ve had to put Steri-strips on my right breast. We’ve been using bacitracin twice a day and covering it with sterile gauze. I’m not sleeping though.. I’ve got less than 7 hours of sleep in the past 72 hours! Insomnia is causing major depression and anxiety :( I went and saw my primary care dr today and they prescribed two meds to help me sleep and with the nerve pain in my hand, then follow up with Dr.Nimtz tomorrow

I am now on day 11 post op. On day 6 the drain tube came out, thank god that thing is hideous. However it did burn pretty bad when Dr Nimitz pulled it out. Day 10 the stitches came out... YaY! The burnt some coming out, but not too horrible. I sill have some swelling on my back and in tummy area, but he said it would take little bit for all the swelling to go away! Hats off to Dr Nimitz and all his wonderful staff! I feel like an actual person.
