Roger K. Khouri, MD, FACS is a board certified plastic surgeon at Miami Breast Center and the leading hand surgeon at Miami Hand Center with over 25 years of experience. Dr. Khouri is a firm believer in minimally-invasive regenerative procedures.
Dr. Khourri is extremely. knowledgeable and creative -- really, an artist. He has developed a unique process for AFT (Autologous Fat Transfer) using very small cannulas that allow him to harvest fat from very low fat density areas of the body. I was a tough candidate for flap options because I had no large area of fat from which to create a flap (BMI ~21) and other surgeons were discussing cobbling together 4-6 donor sites to create one B-cup breast. Dr. Khourri is extremely accomplished with healing massive previous issues. I had a hand-sized hole in my chest due to a debridement of a failed implant and most of the skin on that side of my chest (up to my collar bone) was charred (battleship color) with 0 elasticity due to extensive radiation for Stage IV Breast Cancer. Not only did Dr. Khourri heal the skin (no sign of any radiation damage, currently), but he was able to grow enough surrounding skin to pull it over a cleaned up scar concavity and build a pocket for a breast. I now have a nice B-cup breast (previous size) where the cancer breast with its former, failed implant and reconstruction had been. Besides Dr. Khourri, the other personnel in the office are wonderful - they treat you like family. This is very welcoming and reassuring as the whole procedure (from cancer to successful reconstruction) can be / is a bit traumatic. I have received excellent results compared to previous situation. I couldn't ask for more.
I was born with severe tuberous breasts and have have four sets of breast implants beginning in 1985. I could never get an attractive result due to the lack of lower poles of my breasts. I developed hard capsules and scar tissue as a result of multiple implant exchanges. After my fiftieth birthday, I became aware of fat transfer to breasts. I spent the next seven years researching surgeons from around the world who are skilled in this produce. As a critical-care registered nurse with thirty years of experience, I always perform strict and thorough due-diligence when making medical decisions for myself and family. What I discovered is that very, very few surgeons have the knowledge and skills to successfully and routinely perform fat transfer to normal breasts, much less successfully treat tuberous breasts. I discovered that Dr. Khouri is the go-to surgeon medical professionals use for themselves, family, and friends if fat transfer to breasts is desired. Finally, as I approached my fifty-eight birthday, I scheduled a consultation with Dr. Khouri in Miami. From the moment I arrived at the office, I knew I had made the right decision. All staff members I encountered were friendly and professional. Dr. Khouri spent almost an entire hour with me during consultation. I told Dr. Khouri that my goal was to have normal shaped breasts an be implant free. He answered all of my questions and informed me he would need to perform the RAFT procedure in order to ensure full breasts after implant removal. He also informed me that it wold take two different surgeries in order to achieve maximum success. I scheduled my first surgery for late September. My first surgery occurred September 27, 2018. The surgery included RAFT procedure, liposuction from thighs and hip, and fat injections to both breasts. The surgery was successful with no complications. However, it is imperative to say that this is no for the faint of heart. There was bleeding (mixed with injected fluid) from the lipo puncture holes for five days. There was significant internal stitching associated with the RAFT under breasts and under arms that caused tightness. The overall pain associated with the this surgery was more than I had anticipated. I was also required to wear a breast “splint” for seven weeks to help the newly formed breasts keep their shapes and attach to surrounding tissues properly. Dr. Khouri made two 1 inch long incisions under the armpit to remove my large 400 cc implants. Other than that, there were no other incisions made. The lipo puncture sites are so small that they cannot be differentiated from existing moles. The second surgery occurred on January 10, 2019 and was shorter and less painful than the first surgery. I am posting photos from four weeks post first surgery and 1 week post second surgery respectively. I have no “before” photos of my small tuberous breasts as I’ve had implants for over 30 years. However, I well remember that my bra size was “34 AA” and I didn’t even have enough tissue on the lower poles to keep a bra from sliding up past my nipples when arms were raised. Keep this in mind when viewing the “after” photos to get a full appreciation of just how much fullness Dr. Khouri was able to obtain with the surgeries. It is worth noting that I estimate a 85-90% retention of fat from the first surgery. I literally noticed only a small amount of size difference twelve weeks post first surgery. The second surgery consisted of adding a little more fat to the breasts and removal of all internal sutures associated with the RAFT. Overall, I am thrilled beyond description at my results. I am on track to be a full 34 C (maybe bigger) as I recover from the second surgery. Being implant free has been a dream of mine for decades. I believe Dr. Khouri is the only surgeon who has the skill and knowledge to handle a case like mine and make me whole.
My situation was pretty hopeless when I came to Dr. Khouri. After a bilateral mastectomy with reconstruction (implants), chemo, and radiation, I developed infection and had to have implants removed. There was no good alternative available to me in Dallas. I investigated lat flap and microsurgery but I was not a very good candidate for either. Due to the extreme damage and scarring to my tissue from radiation and infection, I have had to undergo a lot of procedures, but with his very inventive process, Dr. Khouri has rebuilt my breasts with live, healthy fat cells he moved from places where I had plenty of excess (read "oversized thighs" haha) and we are very near completing the process. His medical staff is extremely competent and personable. They feel like old friends to me now. It's been a long journey, but one I am glad I pursued. I was able to avoid large incisions (and associated scarring), rebuilt breasts with my own tissue, and get some nice body sculpting along the way.
In September of 2015, I had a prophylactic mastectomy (nipple-sparing) with implant reconstruction. My experience with the implants was horrible. Not only were the incisions and overall look and feel of the implants way less than acceptable, but my life and overall health started to deteriorate. I started experiencing significant joint pain, overall fatigue, and depression. I was almost at the point of having the implants removed and simply go flat when I found Dr. Khouri. No plastic surgeons in Orlando offered the option to completely reconstruct my breasts using fat transfer. I had three surgeries to gradually remove the implants and exchange them with fat transferred from my legs, back, and abdomen and three more surgeries to correct the shape, size and significant scars from the previous surgeries before I found Dr. Khouri. At the time of the initial mastectomy, the plastic surgeon had removed so much tissue on the side of my breasts causing my nipples to be oddly pointing to each side of my body. As anyone can imagine, this was incredibly unattractive. Dr. Khouri used a RAFT procedure for the first three surgeries to build the fold of the breast and provide shape. I loved the extra bonus of having my upper abdomen lifted as part of the procedure. I experienced some initial pain in the immediate post-op period and it limited full range of my arms, but worth it in the end. For one of the surgeries, I used the Brava system (I believe now called Eve) which uses domes over the breast and suction to help expand the tissue, increase blood flow and allow an environment for the transferred fat to survive. I experienced some skin irritation from this procedure but found ways to manage it with creams and occasionally had to limit my usage to allow it heal. The process worked and Dr. Khouri was able to expand a stubborn area of my breast. The liposuction for fat harvesting was less painful than I imagined. It felt like a sunburn and I experienced significant bruising and swelling but all of it subsided within a few weeks and I loved the flatter stomach, no love handles, and much improved shape to my legs! After the surgeries, I wore a splint to help the breasts keep their shape and promote survival of the transferred fat. I did experience skin irritation with the splints and had varying degrees of number of weeks I was able to wear the splints for each of the surgeries. The longer you wear them, the better the result. At my initial consultation with Dr. Khouri, he provided hope that my breasts could look and feel normal (even attractive) again. He was able to bring the nipples back to center, rebuild my breasts and I am now implant free! I will be having one more procedure to add a bit more volume to my breasts and I am very happy! Dr. Khouri is an extremely skilled surgeon. I came to him with quite a challenge of disfigured breasts. He was determined to give me back what I lost with my initial mastectomy surgeries. My breasts are normal, have sensation and my overall health is so much better! As I mentioned, not only did I get rid of the implants, but my breasts look so much better, and my overall shape is much improved thanks to the added bonus of the liposuction. Dr. Khouri's team (so many people to mention) are fantastic. They are caring people, always available no matter when or for what question, issue, etc. I was treated with such compassion by everyone! I feel very fortunate to have found Dr. Khouri and his team!
Dr. Khouri is a genius and pioneer in his field. In December 2016, I was diagnosed with the earliest stage of breast cancer. 13 days later, I had a double mastectomy with silicone implants all in one surgery. I hated the implants because they felt like foreign objects just beneath the surface of my skin. No fat or tissue existed to make it feel soft and somewhat natural. During my recovery, I researched fat transplants and came upon Dr. Khouri’s website. I didn’t want implants and after 4 reconstructive surgeries in a single year, I’m happy with my natural breasts. The entire process was painful, especially the first surgery which involved the RAFT procedure. But when it was all done, I was very pleased with the result. You can’t even tell I had a mastectomy!! My breasts now are soft and natural-looking and much better than implants. I’m a large B-cup. One bonus from the liposuction is that the fat is taken largely from your back and thighs which made it seem like I had a butt lift without even touching the butt. I highly recommend Dr. Khouri. There are very few doctors who have the expertise that he has - and I’ve witnessed first hand how Dr. Khouri trains other doctors across the country and around the globe who want to know what he’s invented.
History of surgeries: I am a BRCA 1 patient with a very strong family history of Breast Cancer. In May 2016, I had a nipple sparing preventative double mastectomy with expanders and immediate fat transfer. In September 2016, I had a reconstructive surgery with implants and some fat transfer. In March 2017, I had a lymph node removal surgery to remove 6 nodes from the right axilla. In April 2017, I had a second reconstructive surgery to remove the implants and replace them with smaller implants and more fat transfer. In October 2017, I had the third reconstructive surgery to completely remove the implant from the left breast and fully replace it with fat. The right breast had to maintain another smaller implant. April 2018, last reconstructive surgery to remove the small implant from the right breast and fully replace it with fat transfer. Problem: After my reconstructive surgery in 2016, I developed a lump in my lymph node, which after extensive medical exams, we figured out the it was a reaction to the implants. Not only was my body rejecting the implants but they also felt like hard rocks stuck on my chest. I woke up every morning feeling pain. Why Dr. Khouri: Dr. Khouri and his team suggested the best option was to go implant free. Dr. Khouri is a pioneer in breast reconstruction using fat transfer so I chose to move forward with the procedure. I had 3 more reconstructive surgeries to make the implants smaller and adding more fat transfer. Currently, I am implant free and couldn’t be happier with the results. Results: I am ecstatic with the results so far. There is no comparison of implant vs implant free. My breasts look and feel so natural. Even with a shirt off, it is hard to tell that I’ve had so many surgeries. And the added value of having lipo on my thighs and back is amazing. I love how my body looks and though it was a long and painful process both physically and mentally, I am glad that I made the decision to undergo a prophylactic double mastectomy and to choose an implant free option. The staff at MBC are all so amazing all the nurses including Blanca are like family to me! After every surgery, I had at least two nurses that paid close attention to me in the recovery room until
I write this review over 2 years after finishing surgery; most people write reviews early but not me. Had I been writing it immediately afterwards when still sore, probably would be a different story. You are bitter and cranky when in pain. I read a lot of online reviews before starting; all seemed very raw and fresh and hyper focused on a few unpleasant points. Here's my chance to lay it out full picture - and emphasize how long term a process this is. I think they try hard to keep you from being scared and people are surprised that this is way more intense than run of the mill plastic surgery. But it's so worth it at the end once the pain is gone and the scars have faded. To set the picture, age 37 and bilateral mastectomy; I had a DDD chest before that surgery and the TRAM FLAP reconstruction resulted in barely an B cup. I wished to return to pre-cancer size which was proportionate to my height and implants were never an option as I wear historic 16th century corsets regularly and needed natural breasts as implants don't work with that style of clothing. Several surgeons in Denver referred me to Dr. Khouri as the best chance for a difficult situation. I already had a ton of bad scars from the mastectomy, that wasn't a concern. (Dr, Khori actually went back and fixed the worst ones, and now I have beautiful barely visible scars that are hidden by a bikini top.) I had my hesitations as the facility is small and dated. However, the staff are warm and understanding of the trauma people have gone through that brings them to Miami. Dr. Khouri may not present himself as the most politically correct sensitive person for those that are hyper sensitive, but his obvious passion for reconstruction of breasts into the best shape possible can't be missed. He is blunt, but not creepy. Blanca, his nurse, was my primary contact pre and post operative and she was beyond amazing with her compassion and advice. When she gives advice, follow it. Despite the age of the building, it never felt anything other than clean and sterile and I had no post op complications from infections. Office staff comes across as a bit scatterbrained, I kept getting insurance statements showing 'pending' for a year. However, they never asked for a penny more than the initial up front payment for being Out of Network as we kept all surgeries in the same deductible year. I ultimately spent more on travel than the procedure itself. (Mostly because I traveled alone and had to hire a private nurse for the 2 days after surgery each time and relied heavily on courier food delivery. It would be cheaper now thanks to Ubereats.) I had enough breast tissue going in that I didn't get to wear Khour's fancy expander contraption. It wasn't needed. Definitely didn't need it after the first round of surgery as I went up a cup size each time. The surgery recoveries themselves are messy, extremely painful, and the recovery is traumatic. They probably could manage those expectations a bit better. You'll need at least 2 boxes of the big hospital pads, assume you're going to throw away the bed-sheet, and you'll want a dollar store plastic table cloth under the sheet as well as to put on the seat of your car for the drive home afterwards. I don't want to sugar coat how much the recovery hurts and do not be late on taking your pain meds or you will be regretting every movement. However, it's so worth it. I suggest sleeping as much as possible! Don't ignore the instructions to avoid underwear and tight clothes; you'll want to wear mumus. (Out of state people, make sure you fill your Rx early; pharmacies put out of state Rx's through extra scrutiny and Florida pharmacies won't fill a narcotics Rx for someone out of state.) The cast you wear afterwards is glued to you. It's sucks all around and there is no avoiding that. It will itch. It will smell. It will be awful. You won't be able to shower beyond a sponge bath. You'll also have an irresistible urge to want to decorate it like Xena or Wonder Woman because it's pretty much like that, just covered in that flesh tone medical tape that will get super gross looking. You'll have to go back to have it redone once the swelling goes down and it starts falling off. They'll give you an letter for TSA security explaining it for when you fly, but TSA at Ft. Lauderdale Airport barely looked at it - it's common enough. This part is awful because you are slowly returning to normal life. I suggest wearing a sports bra over it to keep you from scratching and picking at it. I really think this was worse than the painful surgery part because it was super annoying, and I slept most of the week after surgery anyway. Taking it off was wonderful, as was going bra shopping at the mall on the way back to the airport. It was very euphoric. I felt so normal each time! Spent so much time looking in mirrors it bordered on mania. Even through it hurt worse than the mastectomy, I went and did this process 4 times; returning every three months. I have zero regrets and pretty much try and forget that year happened. They put in a pain pump after the internal stitch on the 3rd surgery that included an internal appliance to give a defined breast shape, a small section of skin went necrotic due to trauma at the pain pump insertion location. This was revised in my 4th surgery and wasn't a big deal because I followed proper wound care procedure for the entire 3 months between surgeries. Scar is barely visible because Khouri has amazing skill with sutures. So, ultimately, the surgery and recovery process hurts way more than the mastectomy ever did, but now that it's in the past and the memory of the pain fading, I am super happy and delighted with the results. They look great and a tiny bit of sensation has returned; I still get the occasional zing and itch of a nerve regrowing. I strongly recommend this method of reconstruction to anyone wanting to avoid implants so long as they are able to mentally handle it and keep an eye on the end goal. I now wear bikinis and turn heads in public with low cut tight shirts. I don't need a bra because they are perky and firm, but I revel in wearing one - a 36G is my current bra size. A normal appearance helps put the trauma behind me. If you are expecting to go through this and bear zero marks of a breast cancer nightmare, this isn't for you. You need to accept you are a survivor and be realistic about what can be done. But this process of surgery and liposuction will make it so outwardly, people who are unaware of your history will continue to be unaware. Finishing his processes with no belly or back rolls is just a huge perk. You will love that. On the 4th and final surgery he asked if there was any areas I wanted targeted as a fat source besides the 'best' areas - he was running out of donor sites. Dr Khouri, Blanca and Cindy are all kind , intelligent and helpful. All three were there for what i needed and when i needed. Dr Khouri is most definately #1 at this profession. He makes permanately bigger breast with your own fat. He also helps breast cancer surviving patients who had there breast removed grow new breast. Fat transfer to the breast is difficult for the body not to reabsord the injected fat. No other surgeon can do this the way Dr. Khouri invented. I was skeptical in the beginning before using my turbo machine to start expansion. I figured it was worth a shot but just a dream in the far run. After the first use of turbo i was amazed. I took my pics. After 2 more weeks the expansion tripled looking back. The machine itself is worn at night. It comfortably goes on, done gently as it suction is a very soft thick silicone. You wear it nightly with the 'mm' pressure Blanca assigns for you. You can be up and easily move with them but lying down the majority is best. You dont feel much while there on just a little good. After peak the peak hours 10+ you make wake to feel there glued to you this IS a good thing. Once you pull the suction plug they release and expansion ends. The day before surgery is marking day. Very fun because you get to select any areas youve wished fat gone. Mine was partially ankles and calves. From research finding a surgeon to lipo this area is scarce. There reluctant due the complexity of nerves, it can dangerous. Dr Kouri did it and superbly. I now feel great and have maintained two increased cup sizes from 34a to 34c.
I came to Dr. Khouri after having a bad experience with a previous surgeon who had performed a breast reduction. He corrected my breasts by removing implants that were mis-shaping them (although my previous surgeon had placed them there "for the right shape") and also used fat transfer to create natural volume in my breasts without the need for any kind of implant. He also constructed them in a way so that despite being all natural they're perky and upright instead of saggy. Dr. Khouri is an absolute perfectionist. His entire staff was nothing but helpful and supportive every step of the way. I had many appointments and checkups after surgery and each time he and his staff took lots of time with me to check in and see how I was feeling, make sure I was healing well, and make sure I was happy with the results every step of the way. My results are better than I even could have imagined was possible. I love my breasts! I'm so thankful to Dr. Khouri, Blanca, Cindy, and the rest of the team at Miami breast center for their unparalleled level of professionalism. I would recommend them to anyone who is serious about finally fixing their breast issues. They are worth it!
My story began in February 2015 when some suspicious calcifications were found during my annual mammogram. Then, few weeks later, I received biopsy results with confirmed diagnosis: cancer in my left breast. Then numerous tests, office visits, consultations, and ultimately surgeries. My breast surgeon wanted to preserve my breast thus he offered to start with lumpectomy. Well...I had three. You couldn’t possibly asked for better looking incisions. Almost as I never had anything. From cosmetic point of you. However, it was impossible to get clean margins. Therefore, I had only one choice- mastectomy. I opted for double, right side as preventive. While I had this odyssey with lumpectomies I started contemplating potential reconstruction. I rejected the idea of implants immediately. Then the only choice left was DIEP reconstruction which was not an option either once I learned the magnitude of the surgery itself and its recovery afterwards. I remember the moment when I came across the name of plastic surgeon in Miami, Dr. Roger Khouri, who was doing breast reconstruction by autologous fat transfer. The idea that you will carry your own fat taken during liposuction instead of cold foreign objects as implants seemed more than tempting. Upon my request, my local breast surgeon talked himself to Dr. Khouri the day prior to my scheduled double mastectomy. Dr. Khouri instructed him what should be done during the surgery in case I pursue fat grafting reconstruction in Miami later. What a team of doctors I’ve got. Being both extremely busy and very well world known, they found the time to discuss my case. How often does that happen these days?.... Well..... few months later, in September 2015, two months after healed mastectomy, my Miami story began when I first met Dr. Khouri. Plastic Surgeon with brilliant mind, golden hands and compassionate heart! An artist with constant desire to change and improve! But most importantly, to help women to look as natural as possible after breast cancer surgeries/treatments. The doctor who is passionately in love with his work. Dr.Khouri developed the plan for my surgeries assuming there will be 3-4 total. My first was in September 2015. Dr. Khouri sculpted my breasts by placing RAFT, adding a small implant which had to hold the shape and filling the rest with my own fat. Since then my Miami rhapsody began. My body refused to cooperate. The implants were rejected from my body, even after trying the second time with a smaller size. My own body just decided to ruin Dr. Khouri's original plan. So what would any well-known, respectable arrogant surgeon would say to the patient? -Sorry, but your body just can't handle it, please opt for DIEP reconstruction. Yeah, any arrogant surgeon. But NOT Dr. Khouri. He started thinking of something else, how to sculpt my breast with having no support from anything. The new chapter began and numerous surgeries as well. Pain, disappointment, lack of hope were my friends back then. But not a single thought of quitting. Same with Dr.Khouri. Always cheerful, positive, with contagious laugh, creative, exploring fresh ideas and inventing breakthrough techniques. Always thinking of something new, something better..... To be fair, I was very compliant patient, listening and doing every little thing I was instructed. I was wearing Brava, trying to do everything possible to achieve maximum expansion (though my body didn't help me much with that either). I've had 8 surgeries total. The last one was in November 2017. As of now, I am looking at myself and still can't believe what I see. Can't really comprehend how it was possible. How surgeon's magic hands and deep desire to make it work, could deliver that.... To say I am grateful is not to say anything. Dr.Khouri, ONE and ONLY, with his vision, his skills, his tenacity (I’ve seen him starting last surgery @ 8pm, having first one @ 9:00am) and ultimately his personal mission to help women to get their life back, looking as good as possible. Well....And it is another person in Dr. Khouri's office. Blanca Lara. His registered nurse, his right hand. I can’t even tell you how many times I called her, FaceTime, texts, emails....Her replies were unbelievably quick. She is knowledgeable, professional, kind, very caring, warm person. In the most challenging moments of this journey, Blanca has always been there for me with her expert advice and moral support. So here I am now. 3 years and 8 surgeries later. I don't know how I will look in a month, half a year, years from now. Most likely the breasts will change alone with my body changes. It still hurts as my skin, tissue and muscles continue their recovery and adjustment to their new shape and functions. But I feel that I did the right thing which would never happen if there is no Dr. Khouri in this world.
Hello dear. You definitely have hope! The tubular breast is the result of a congenital band around the base of the breast. This impairs normal breast development. However, your breasts CAN be corrected using a combination of techniques that first would release the band around the base of the breast, which is probably causing the "ball" shape that you mentioned. Because of the additional sagging resulting from breast feeding, a breast lift will be needed along with fat grafting for additional volume. I strongly urge you to seek a consultation with a board certified surgeon that has experience with breast deformities. Best of luck!
This is an excellent question and one that isn't always addressed properly because it is a more complicated case. There is a new technique called a RAFT (Reverse Abdominoplasty with Fat Transfer), which advances a significant amount of tissue of the upper abdomen into the breast to add volume to the breast and define the breast fold. In essence, it's like reconstructing your breast fold. With the RAFT we slide up the lax abdominal tissue into the breast without any incision. This is like an abdominoplasty (tummy tuck) in reverse as we pull the tissues up instead of down like we would in the standard abdominoplasty. Using only needle puncture nicks, the PALF maneuver allows us to advance the abdominal tissue into the breast without having to cut it out and transfer it as a flap. The tissue brought up from the abdomen serves to provide the skin envelope needed to better contour the breast and to better define the inframammary breast fold. It is also an additional recipient matrix for fat grafting that can provide more volume and help fill contracted areas. This technique has been used to correct breast deformities, both congenital and reconstruction-related. For the best outcome, you should be seen in consultation, preferably by a board-certified plastic surgeon and one who is experienced with this revolutionary technique.
Unfortunately there are no magical medicines, oils, nor creams that will give you a 1-2 cup size breast augmentation. But the least invasive breast enlargement surgery is a fat transfer breast augmentation. Based on published clinical studies, this procedure is highly safe and effective. And it is the most natural looking, natural feeling alternative to implants. Your body will not reject your own body fat (autologous) and the augmentation achieved is permanent. It's just like transferring the fat from one place to another. If you're concerned with hazards that sometimes accompany implant breast augmentations, you should research and consider a fat transfer breast enlargement surgery, which uses your own fat and an external expander prior to surgery. But make sure you see someone who is experienced with this technique, as results vary if not done exactly as in the published articles.
It's hard to say how much extra volume the fat grafting will give you. It depends on the plan your surgeon creates for you and your needs. Sometimes fat is just placed around an implant to give it a more natural look, however, it does make the breasts look larger. Some women replace their expanders entirely with fat grafts only. So it all depends on how much fat your surgeon will transfer to the breast area based on how large you want to be. Be wary of going too large all at once, especially if you've had radiation, which you haven't specified. Again, so much depends on your individual needs. Make sure you choose a board certified surgeon and one experienced with fat transfer to the breast for the best results. Best of luck!
The most natural option to downsize your implants would be to replace them with your own fat. The implants can be removed from the same incision in the armpit and then the breast area can be immediately filled with fat grafts that are liposuctioned during the same surgery. You will end up a lot smaller than your implant and the tissue will naturally shrink back some. If you don’t want to be too small, a smaller implant can be inserted with fat around it. The breast lift and aerola reduction should be discussed in consultation. There are several options depending on how large/small you want to end up.
