Amazing Experience with Dr. Roger Khouri’s Breast Reconstruction with Fat Grafting Post Double Mastectomy

History of surgeries:
I am a BRCA 1 patient with a very strong family history of Breast Cancer.
In May 2016, I had a nipple sparing preventative double mastectomy with expanders and immediate fat transfer.
In September 2016, I had a reconstructive surgery with implants and some fat transfer.
In March 2017, I had a lymph node removal surgery to remove 6 nodes from the right axilla.
In April 2017, I had a second reconstructive surgery to remove the implants and replace them with smaller implants and more fat transfer.
In October 2017, I had the third reconstructive surgery to completely remove the implant from the left breast and fully replace it with fat. The right breast had to maintain another smaller implant.
April 2018, last reconstructive surgery to remove the small implant from the right breast and fully replace it with fat transfer.
Problem:
After my reconstructive surgery in 2016, I developed a lump in my lymph node, which after extensive medical exams, we figured out the it was a reaction to the implants. Not only was my body rejecting the implants but they also felt like hard rocks stuck on my chest. I woke up every morning feeling pain.
Why Dr. Khouri:
Dr. Khouri and his team suggested the best option was to go implant free. Dr. Khouri is a pioneer in breast reconstruction using fat transfer so I chose to move forward with the procedure. I had 3 more reconstructive surgeries to make the implants smaller and adding more fat transfer. Currently, I am implant free and couldn’t be happier with the results.
Results:
I am ecstatic with the results so far. There is no comparison of implant vs implant free. My breasts look and feel so natural. Even with a shirt off, it is hard to tell that I’ve had so many surgeries. And the added value of having lipo on my thighs and back is amazing. I love how my body looks and though it was a long and painful process both physically and mentally, I am glad that I made the decision to undergo a prophylactic double mastectomy and to choose an implant free option.
The staff at MBC are all so amazing all the nurses including Blanca are like family to me! After every surgery, I had at least two nurses that paid close attention to me in the recovery room until

I am hoping for the same outcome - my surgery 7/13/18 and 6 weeks post op, still have one of the drains in my abdomin and it continues to clog retaining fluid, that builds up, then my abdominal incision has had internal pressure, causing it to open up a bit. breast tissue on left breast has not healed well, have open wounds along outside incisions both to and bottom and just want to be able to feel better. Youve certainly gone thru so much - so you give me hope. Thank you!

Hi @kncorrell! Did you get the procedure done with Dr. Khouri? I only had the drains when I got the double mastectomy at a different hospital.

I’m so happy to hear about your success and excellent recovery. You look amazing! I had a double mastectomy in 5/2012 with a lat flap reconstruction that was not aesthetically pleasing. Fortunately they got all the cancer out. Just 2 days ago 12/11/2018 I decided to revise the reconstruction and am so happy I did. I love the Dr. I used, he was patient and understood what I wanted. I wanted to look natural and feel like I hadn’t don’t thru so much. He added some fat grafting from My thighs to fill the hallow at the top of my breasts. I had felt very wide - before - turns out it was the lat muscle and not extra fat. Dr. Mahabir severed the nerves or something like that so they wold shrink and I wouldn’t have that fatty looking bulge, nor would I have the deformity every time I lifted myself up with my hands out of a pool. Question, did you ha to wear Compression garments from where the fat was grafted? I am unclear how tight they should be. I will prob ask surgeons office, but I feel like I call them too much lol

Hi. You're reconstruction looks amazing. I am currently scheduled for a Mastectomy of my right breast only. I contacted Dr. Khouri's office and had a consult with him via FaceTime. I would very much like to ask a few questions. If you have a moment would love to hear from you. Did you use the external expansion system he has? Or was the procedure all done with internal expanders/implants? If it was internal was the expander placed under the muscle or on top of it? Seeing your photos has been very inspiring especially since it has been so recent. I have searched and researched and Dr. Khouri is really the best from what I have read and seen in reconstruction photos. I have an autoimmune disease and I am very concerned about having an implant long term in my body. Thank you for sharing your journey.

Hi @camschee! I chose to go with inter expanders and implants but I wish I had just done EVE. The internal expander was defected and it was leaking so my right breast still has a weird shape because of it. Also the implants made my lymph nodes abnormally swell, the doctors thought it could be cancer so I had to get my lymph nodes removed.
I used the EVE for my last surgery and it was totally doable and manageable!!

Hi @chieisa. Thank you so very much for taking the time to respond. It is really helpful. I have been scrambling to get everything in order pre-mastectomy. I am relieved to hear the EVE is doable and manageable. I am really nervous to have the expander because I have no idea how my immune system with an auto immune disease will respond to the expander. I am going to consult with Dr. Khouri again and discuss the EVE.
Can I ask was using the EVE painful to just uncomfortable. I had read of some people getting blisters. Thank you again. I am grateful for your sharpen your experience and journey. It is truly helpful.

No worries, I understand I was on the same boat not long ago. The EVE is not painful at all. It's important to stay hydrated while you use it!

hELLO This is very helpful. I have breast cancer and have had a mastectomy and am looking at reconstruction DIEP surgery soon. I had noticed that this patient has a transparent kind of gauze possibly forhealing. What is this gauze and can ipurchase some for when I have my operation?
