I was born with severe tuberous breasts and have have four sets of breast implants beginning in 1985. I could never get an attractive result due to the lack of lower poles of my breasts. I developed hard capsules and scar tissue as a result of multiple implant exchanges. After my fiftieth birthday, I became aware of fat transfer to breasts. I spent the next seven years researching surgeons from around the world who are skilled in this produce. As a critical-care registered nurse with thirty years of experience, I always perform strict and thorough due-diligence when making medical decisions for myself and family. What I discovered is that very, very few surgeons have the knowledge and skills to successfully and routinely perform fat transfer to normal breasts, much less successfully treat tuberous breasts. I discovered that Dr. Khouri is the go-to surgeon medical professionals use for themselves, family, and friends if fat transfer to breasts is desired. Finally, as I approached my fifty-eight birthday, I scheduled a consultation with Dr. Khouri in Miami. From the moment I arrived at the office, I knew I had made the right decision. All staff members I encountered were friendly and professional. Dr. Khouri spent almost an entire hour with me during consultation. I told Dr. Khouri that my goal was to have normal shaped breasts an be implant free. He answered all of my questions and informed me he would need to perform the RAFT procedure in order to ensure full breasts after implant removal. He also informed me that it wold take two different surgeries in order to achieve maximum success. I scheduled my first surgery for late September.
My first surgery occurred September 27, 2018. The surgery included RAFT procedure, liposuction from thighs and hip, and fat injections to both breasts. The surgery was successful with no complications. However, it is imperative to say that this is no for the faint of heart. There was bleeding (mixed with injected fluid) from the lipo puncture holes for five days. There was significant internal stitching associated with the RAFT under breasts and under arms that caused tightness. The overall pain associated with the this surgery was more than I had anticipated. I was also required to wear a breast “splint” for seven weeks to help the newly formed breasts keep their shapes and attach to surrounding tissues properly. Dr. Khouri made two 1 inch long incisions under the armpit to remove my large 400 cc implants. Other than that, there were no other incisions made. The lipo puncture sites are so small that they cannot be differentiated from existing moles. The second surgery occurred on January 10, 2019 and was shorter and less painful than the first surgery.
I am posting photos from four weeks post first surgery and 1 week post second surgery respectively. I have no “before” photos of my small tuberous breasts as I’ve had implants for over 30 years. However, I well remember that my bra size was “34 AA” and I didn’t even have enough tissue on the lower poles to keep a bra from sliding up past my nipples when arms were raised. Keep this in mind when viewing the “after” photos to get a full appreciation of just how much fullness Dr. Khouri was able to obtain with the surgeries.
It is worth noting that I estimate a 85-90% retention of fat from the first surgery. I literally noticed only a small amount of size difference twelve weeks post first surgery. The second surgery consisted of adding a little more fat to the breasts and removal of all internal sutures associated with the RAFT. Overall, I am thrilled beyond description at my results. I am on track to be a full 34 C (maybe bigger) as I recover from the second surgery. Being implant free has been a dream of mine for decades. I believe Dr. Khouri is the only surgeon who has the skill and knowledge to handle a case like mine and make me whole.

Thank you for your post. I will be posting my journey with Dr Khouri as well. Question for you...I have had Lipo previously so I know what to expect as far as the painful recovery from that aspect. I am curious what my chest will feel like. I suffer from anxiety and am very nervous the pressure in my chest with cause a panic attack. I would greatly appreciate your input.

To be honest, the chest/breast part of the surgery has been the least painful part of both surgeries. The pain medication prescribed was most effective with the discomfort associated with the breasts as opposed to pain associated with the RAFT/lipo. With the first surgery, the RAFT stitching underneath the breast was the most uncomfortable part. I felt like there was a constant “pull” on my upper abdomen and I couldn’t get complete relief from it no matter what I did. It is also very important to mention that my arm range of motion was very compromised after the first surgery due to the internal twitching associated with the RAFT. This has greatly resolved since the internal sutures have been removed. The lipo sites were also more painful than the chest area. I weighed 114 lbs at 5’4” tall on surgery day so you can imagine what a challenge it was for Dr. Khouri to extract enough fat for the procedure. I am at the healing stage now (post second surgery) where I am feeling sharp shooting pains through my breasts intermittently throughout the day. I attribute this sensation to possible healing nerve tissue. I also experienced this about three weeks after the first surgery. These stinging sensations last about 1-2 weeks. I can’t describe these as painful, but rather annoying. I hope this helps. Please do not hesitate to ask more questions. I will answer as fully and truthfully as I can. Thank you so much for replying. I really appreciate it! I won’t be if having the RAFT procedure, only Lipo and fat transfer and of course I have to wear the EVE prior for 3 weeks. Did you have to wear the expander? If so, how unfortable is it? Did you feel pressure or tightness in your chest after the second surgery? And as far as the sharp pains in your breasts just to make sure I’m understanding what you said...they started about 3 weeks after the fat transfer and last about 2 weeks? Was it all day long?

I was not required to use the EVE device prior to surgery. Since I had 400 cc implants removed, I already had a large stretched-out void in the breast region so no need to wear the expander. The major challenge with my case was to be able to move abdominal tissue to the breast region in order to create a scaffold/matrix in which to inject fat. Since I had virtually no natural breast tissue (Dr. Khouri confirmed this, especially after the first surgery) Dr. Khouri greatly relied upon the RAFT to achieve great results.
As far as the feelings in the chest area after the second surgery, I didn’t have anything that could be described as “pressure.” I did feel some tightness, especially when lifting my arms or reaching for an object. This tightness continues but is much less than immediately after surgery. Perhaps this was due to soreness of the pectoral muscles??? The sharp pains I referred to were/are more like a pin prick - a sharp, sudden, almost electric pain sensation that lasts for about one second each. These sensations may come in clusters of 4-5. These pains are/were not constant but intermittent -maybe 2-4 clusters per hour - 5-6 hours per day. I am much more aware of the pin prick sensation when I am settling down for the night. Ibuprofen and 1/2 Percocet at night completely alleviates these pains. And yes, the chest/breast sharp pains were not immediately after surgery - they began about 2-3 weeks afterwards. The sorest lipo area- by far- was the love handles area on my lower back. This made it very difficult to lay in bed for the first week. I actually elevated my butt cheeks on a small but firm pillow in order to keep my love handles (lower back) from making contact with the mattress in order to alleviate pressure. This strategy worked well. As far as fat retention goes, I am not at a point to where I can make an overall accurate assessment. I am still swollen over most of my back so I’m not sure what that final look will be. My abdomen is flat and very smooth and even - I am very pleased with this. My back is somewhat lumpy and uneven now but I am less than three weeks out of the second surgery. The back area is where most of the fat was harvested for the second surgery. As a runner and avid gym rat, I was very fearful I wouldn’t have enough fat for the procedure. I have been a true size 2 for years and I returned to Miami for the second surgery wearing my size 2 skinny jeans so my actual clothing size has not changed. I will provide updates as the weeks go by in order to provide an accurate picture of what should be expected with this surgical process.

I apologize for the bombarding of questions haha but one more thing, Did the Dr to a nice job of lipo’ing the areas so they are smooth and even?

Yes, I am very smooth on the initial lipo areas. It’s been over four months since the first surgery and the lipo areas are very smooth and even. This includes abdomen, inner thighs, and tops/fronts of thighs. The second surgery mostly included lipo from the back and this area is still swollen and a little uneven looking. I feel it is too soon to judge the quality of the back lipo though. I looked a little uneven and swollen on the initial lipo areas too, but I really started noticing smoother quality at about week 4-5 after the first surgery. I will provide updates as time goes on. I plan on providing more photos too. I welcome questions - this surgical procedure is serious, expensive, and potentially life-changing. All potential patients should be armed with as much info as possible in order make the right decision for themselves. I hope I can help others proceed with confidence by providing an accurate accounting of my journey.
