Hello. There are several different implants out there on the market. If you are worried about silicone due to the fact that they could rupture there is a new "gummy bear" implant that is made of a cohesive gel. In the case of an implant rupturing the gummy bear implant stays cohesive and the idea is silicone wont travel outside of the breast cavity. Although silicone implants are recently the more popular choice, you should find an implant that is best for you. In physician selection you should only use a plastic surgeon who is board certified. Your plastic surgeon should also be able to provide you with the best education and information when making your decision on implants. Best of luck to you!

Tissue expanders are usually placed to assist with the breast reconstruction process. Tissue expanders are gradually filled until you are happy with the volume and size, and they are replaced with a permanent implant of your choice. Typically patients who have had breast cancer are able to still get a reduction and or lift. Depending on the type of cancer, treatment, and clearance from your breast oncologist would determine how soon your breast reduction/lift would be able to take place. Typically patients who undergo breast reconstruction with silicone implants are always under general anesthesia. These cases can sometimes be long and uncomfortable for the patient if they are under a conscious IV sedation. I would strongly recommend that when you do have your reconstruction surgery it is under general anesthesia with a credentialed and certified anesthesia provider. Typically patients who have had breast reconstruction with implants are able to go back to their normal routines once they are fully healed. This would include things such as weight lifting and playing the violin. Hello! If you are unhappy with the size of your breasts a breast reduction is a good option. As far as a complete mastectomy due to a concern with a family history of breast cancer I would advise that you do genetic testing like BRCA. BRCA is a blood test that can give your more information on your risk of breast and/or ovarian cancer. I would check into your insurance, sometimes the BRCA test is covered. Recently these genetic tests have become more affordable if your insurance does not cover it. Once you have your genetic testing you can consult with a board-certified plastic surgeon to discuss your best options. I hope this helps. Best of luck to you!

Typically patients who are 17 months post-op should not be experiencing tenderness or pain. This could be a capsular contracture, but is definitely abnormal and needs to be addressed. My recommendation is to consult with a board-certified plastic surgeon and get a thorough assessment. If it is a capsular contracture you have different options as far as treatment. Of course, it is always a good idea to "contact your operative (board certified plastic) surgeon" with such questions. I typically tell my patients that they can resume physical activity at 2 months postop - if healing is straightforward and they are without nutritional problems. I encourage my patients to go back to very light exercise with a slow and very gradual build up. I tell them, "if it hurts don't do it - especially for the first 4 months". Usually by 12 weeks postop, the wound has 80% tensile strength of your original skin and soft tissues.

Better photos would help. It looks like you need a lift, at least a circumareolar - but probably much better with a vertical scar (lollipop). Ultimately, a physical exam should be performed by an ABPS plastic surgeon prior to any decision.
More helpful picture (clinical breast photo) suggestions:
1) Have a friend help you - don't take them yourself. Stand straight. Photos should be taken as "level" as possible
2) Photo from the bottom of the chin to the umbilicus (belly button). Include full shoulders and arms down or behind your back
3) Good Light
4) Multiple angels. Mostly we want to see two good laterals from each side (standing straight looking from the side - otherwise your profile) and a good photo from the front. Obliques (or 1/8th turn if you ever took ballet) and arms up photos also help!

I had Lipo and a breast lift (no implants) with dr lampert 1 month ago. I’m so happy I found Dr. Joshua A. Lampert! I did research for 4 years to find a good surgeon who can remove my textured implants and downsize then and exchange for smooth ones,I had so many consultations with another surgeons they all forced me to do just explant with breast lift and leave capsule there! I was really disappointed after all these consultations because surgeons did not hear me and did not care about my concerns. Necause explant with breast lift wasn’t an option for me because of my job I have to keep them big size and I can not have scars for now plus for me was the more important part to remove texture capsule! I absolutely know I made the right choice when I chose Dr Lampert and his team. I never felt rushed during my consult and everyone was quick to address any questions that I had. I am so thankful to Dr. Lampert and his amazing staff!! For several reasons I needed Breast explant surgery with capsulectomy, I had many concerns about the surgery, how would I look after, recovery, risk, scaring and so on... All I can say is that both my husband and I are so happy we chose Dr. Lampert as my surgeon. As soon as we entered for our consultation we felt comfortable, his staff was so professional and welcoming and during the consultation he spent so much time with us. I went to him after having a nightmare surgery with Dr. Leonard Hochstein. Thank God for him, my scars are non-existent and I feel so much better!! I would recommend him to anyone. I had implants put in around 23 years ago. Young and dumb, I actually thought they lasted forever. I wasn’t told that you needed to remove or replace every 10 to 15 years. Well mine became very painful, particularly after a mammogram. Crushed, more painful, and hard as a softball. Previous unfortunate encounters with doctors and anesthesia had me scared to do any surgery.It also took me years to find a surgeon who would remove only and leave me with something acceptable. Most of them want to sell you another set of implants. Not an option, I never want them again. This surgeon actually specializes in removal and reconstruction of what I had left. I was unusually comfortable with him and his staff. He spent a lot of time with me, videos, before and after pics, a lot of background questions, the works.I finally did it at their surgery center a few weeks ago. All went perfectly, just as expected. It is a tremendous relief to have done something that I waited so long to do. I feel such gratitude towards Dr. Lampert and his entire staff, words cannot express. It’s been a few weeks and already the difference is quite apparent.I can’t believe I lived with those things in me for so long. I also didn’t mention the fibromyalgia I have suffered with for years, that might have been aggravated by the implants. Any improvement there remains to yet be seen. After years of pain and discomfort all I wanted was to get them out! And possibly repair the damaged tissue all around them. Finally, I found these people. Dr. Lampert is heaven sent.
