Thank you for your question. Treating alar retraction can be a little challenging as you are asking to place tissue and length to a part of the nose where it is difficult. I have found that from a non-surgical perspective, fillers of any kind, properly placed, can yield a nice difference. From a surgical perspective, now you look at rim grafts, or caudal extension grafts with tissue usually taken from the ears. Whatever you decide, seek the advice of a dual board certified Facial Plastic Surgeon.

This topic is near and dear to me as I perform anywhere from 6-8 micro-autologous fat grafting procedures weekly and by far the most common area is the tear trough and upper cheek region. As the face ages, there is a tendency to hollow in the area of the tear trough due to thinning and atrophy of the SOOF (sub-orbicularis ocular fat) pocket. This can cause dragging of the lower eyelid ultimately lead to ectropion. The PROPER placement of fat actually helps to support the lower canthus (eyelid) and keep the eye from developing into ectropion. It is very important that you seek the advice of a dual board certified Facial Plastic Surgeon.

Thank you for your question. I first can tell you that revision rhinoplasty is challenging even for the most experienced of surgeons. Under no circumstances should you go to anyone whom is not double board certified in Facial Plastic Surgeon. You need a specialist in the face and nose. Having said that, the goal of revision many times is to add back to the nose. The manner that one does this varies from case to case.

I would speak with your surgeon. It is not uncommon for the nose to look over-rotated early on and settle in. As I do not specifically know what was done, I cannot comment on when the nose will drop if it will. As a rule, I do not like to perform any kind of revision until the nose is soft again. Your surgeon may be different.

Thanks for your question. It appears from the pictures that you may have over-resection of the bridge causing an inverted V deformity. This can be improved with narrowing of the bones, but has to be done correctly. If you feel that you are not comfortable with the surgeon, please make sure that you consult with a dual board certified Facial Plastic Surgeon whom specializes only in the face. Best of luck!

Hello. thanks for your question. I see what your concern is and it is visible. There are a couple of ways that you could correct this. Removing more hump by shaving is an option, and probably the most obvious, but you have to be careful of over-resecting the bridge. This can scoop the nose too much. A second option is to conservatively rasp the hump and graft below so that you remain with a strong bridge. Yet another option is to have your surgeon first put in a filler in the dip below to see if you like the appearance first. When one deprojects the tip of the nose, there has to have occurred a few moves that will rotate at the same time. Removing the hump at the same time and treating the "tension tip" will give that nose an overall balanced aesthetic look. Make sure that you are consulting with a double board certified specialist in the face.

Thank you for your question. I would highly recommend waiting until after the rhinoplasty before you have any lip injections. Rhinoplasty will alter the appearance of the lips for quite some time and can give the injector a false appearance.

This is a procedure that I personally perform 5-6 times weekly and have found that the best remedy is conservatively removing fat pockets and placing harvested fat grafting in the tear trough region. This will give outstanding results. I personally do not prefer to do fat repositioning as this will take from one area to fill in another.

Booked my neck lift lower face lift I took fat transfer surgery for the beginning of October. So excited and ready. 62 have lost the bottom of 140 pounds between age and the weight loss.
