 Hi, "button" chin implant is a term describing a smaller version of a curvilinear style chin implant that's still available.  These styles, in my humble opinion do not offer the same degree of aesthetic improvement found ion the extended anatomical implants.  

  I have performed many facial shaping procedures, including Chin Augmentation with dermal fillers or silastic chin implants, for over 30 years.  When the chin is weak, this creates an imbalance making the nose appear larger, the mid face top heavy, the lower face looks short, de-emphasizes the lips and allows early formation of a "double chin".  

 Proper placement of a silastic chin implant adds forward projection to the chin thereby creating harmony and balance to the lower face.  Using the same incision, liposuction can be performed to reduce the fat and further shape the neck.  Excess skin, from below the chin, can also be removed through the same incision.  

 I have found that placement of a silastic chin implant, through a small curved incision under the chin (also allows excess skin removal) to be very safe, quick, highly effective and far less invasive than a sliding genioplasty (requires extensive tissue dissection, bone cuts and placement of metal screws and plates to secure the cut segments of bone).  I perform chin implant surgery in 30 minutes or less, often using a local anesthetic alone.  

 When consulting with surgeons perhaps ask not only is your chin weak but do they have experience performing chin augmentation?  The attached video shows how I determine whether the chin is weak.  The second part of what that surgeon said is absolutely correct.  The selection of the proper size, shape and thickness as well as the proper placement of the silastic chin implant are all "key" elements in creating a naturally looking chin.  If too large of an implant is used in a woman, the chin can look relatively masculine.  That's where experience, performing chin implant surgery, comes into play.

 Hi, at 7 days it's just too early to make an assessment of the final result.  It would be best for you to express and discuss your concerns with your chin implant surgeon.  The swelling is usually gone (or most of it is gone) at 1 month post op, so you still have 3 more weeks.  In my humble opinion, give yourself enough time to heal and see the final result.  If an intra-oral approach was used, the added tissue dissection will likely increase the swelling and healing time.

 Hi, I am sorry but the limited photo doesn't show enough of your face and jaw line.  Non smiling, resting photos of your face from the front and side would help in the evaluation.  I have performed many SMAS facelifts for over 30 years and have performed many minimally, invasive SMAS facelifts.  The reality is, there are many different ways to perform a facelift from incision length, incision placement, level of tissue dissection (skin only, SMAS, Deep Plane, Subperiosteal), different degrees of tissue undermining, how to lift and support (imbrication versus plication of the SMAS layer), how much excess skin to trim, is excess fat going to be reduced and finally will the overall shape of the face be made more feminine or masculine (while avoiding the over pulled, windswept appearance).  Price is determined by the reputation, location and experience of the Facelift Surgeon.

 In my humble opinion however, there are some basic requirements that must be met in order for a facelift to be "effective", yielding "aesthetic pleasing" and "naturally attractive" results.  These include: 

 1. Some type of incision either curved around the front (preferred) or straight in front of the ear.  This is how excess skin is removed which is not possible with an incision behind the ear alone.  If you have so little excess skin on the face that "none" needs to be removed you are not a candidate for a facelift in the first place.

2. The facelift must address correction and softening of the "jowls" using a long lasting and effective technique.  If you have "jowls” these are sagging facial tissues and the main indication for some form of a SMAS facelift.  The underlying SMAS layer, of the face, must be dissected, lifted, trimmed and re-sutured (not merely folded or suspended with threads or sutures that will not last).  The excess skin is then removed and the facelift incisions closed.

3. Facial shaping must be combined to properly shape the cheeks, chin, jaw line and lips for feminizing or masculinizing the face as the case may be.

 If you have "jowls” these are sagging facial tissues and the main indication for some form of a SMAS facelift.  The underlying SMAS layer, of the face, must be dissected, lifted, trimmed and re-sutured (not merely folded or suspended with threads or sutures that will not last).  The excess skin is then removed and the facelift incisions closed.

 Hi, in my humble opinion local anesthesia and IV sedation are great for a minimally invasive SMAS facelift that takes about 90 minutes.  4 or 5 hours of surgery is a bit long for that type of anesthesia and in that case a general anesthetic might be more appropriate.  Anesthesia, like a facelift is performed in a wide range of methods.

 Hi, I have performed many facial shaping procedures using dermal fillers, facial implants (cheek, chin), liposuction and/or facelifts for over 30 years.  Following my beauty principles, men look chiseled and handsome with angularity in the cheeks, chin and mandibular angles.  From the photos, your cheeks are flat in the front, the chin is weak as are the mandibular angles.  The following procedures would create a more ruggedly handsome face.

  • Cheek augmentation with dermal filler or silastic cheek implants to add angularity to the cheeks and mid face.

Wonderful Experience with facial fillers

I went to Dr. Palmers office a few weeks ago for a consultation regarding my jawline and sagging in jowls. I was greeted very warmly and felt comfortable right away. Dr. Palmer and his wife who also works there, are both very knowledgeable with respect to procedures and fillers. They showed me many before and after photos and answered all of my questions. After the consultation, I decided on filler in my cheeks lips and chin area. The results were absolutely amazing! I felt like I was 10 years younger. Also the prices are very reasonable for filler. Dr. Palmer and Robyn are truly wonderful. As a 53 year old male, I never thought that cosmetic enhancements were my thing but it does make a difference. After discussing my needs with Dr. Palmer, I decided to go with his procedure and I couldn't be happier. Dr. Palmer is honest, professional, and really makes you comfortable. Dr. Francis and Robyn saved me. In 2015, I had chin augmentation and lipo of the jaw line. I could have been on botched. The surgeon (who I won't name) placed a size EAC Large without trying on or showing me the implant ahead of time and removed an unheard of amount of fat. The implant stretched and damaged my muscle and smile. The damage between the lipo and implant was so bad, the top surgeons in the world turned me away. In 2016, I finally found someone to replace some the fat that was removed but he was uncomfortable replacing the implant with the smallest available one because of the damage and pocket size. I suffered and I waited another year, shopping for a surgeon would take me under their wing. I spent so long researching and I finally found Dr. Francis Palmer. Robyn, his patient coordinator invited me in to talk with her, after spending nearly an hour with me on the phone discussing what had happened. It's LA... not many people care...but, she did. She really cared. She invited me in and we chatted while I waited for Dr. Palmer. She gave me an emergency appointment with no consult charge. The three of us talked about all my options and Dr. Palmer knew immediately what I wanted and needed... AN EAC SMALL chin implant because the pocket had to be filled and I had such a small petite face. He joked with me and made me feel like... how my mom or dad would as a kid if I fell off my bike and hurt myself. He knew he could help fix the problem. We made the appointment for the surgery and they asked me to come in AGAIN, just to clarify everything, try on and talk. I decided to do the chin implant removal & replacement under local because he is the best at this. Minimal damage, no pain, it's done in 30 minutes and you can drive home. I didn't feel a thing! I cried a bit out of nervousness because my first experience was so traumatic, and he stopped briefly to pat my arm and tell me he was right there, how tough I am, how well I am doing and how great it looks and he wouldn't let anything bad happen. This is the surgeon you need to see. It's LA and surgeons only care about your money. I have met with over 20 surgeons and Dr. Palmer is the only one I trust. He is genuine and has the best bedside manner and he didn't turn me away. He practices because he cares... he does reconstructive because he knows he has the skills to help... I am still blown away. I thought I would be stuck "botched" forever. I am so lucky to have found Dr. Palmer and his team.

The Recovery Time Is Nothing Less Than MAGIC

Did a lot of research meeting 4 surgeons before finding Dr. Palmer; he offered something completely different from any other Doctor a "Mini-Incision Facelift." Ten days after the surgery no one knew a thing; there was almost no bruising with one black & blue spot on my neck. It really was incredible how fast I recovered. You don't have to stop your life and hide for a month - I am 60 and now I look 50! A special shout-out to his "Wonder Women." When I decided I wanted my lips to be a bit fuller, I asked a few friends who they recommended. All three of the girls that I asked swore by the Palmer's practice and now I do as well! I have been told that other plastic surgeons try to rush you in and out of their office if you are getting a simple procedure such as injections in your lips; the Palmers spent over an hour from start to finish answering all of my questions! You are truly amazing; However, it is not just how you gave me a beautifully lifted face using only restylane filler, but your integrity and superior code of ethics that stands out above any other physician I have ever consulted for cosmetic procedures. I went to see Dr. Palmer for a revision of neck lift surgical scars, and blephoplasty. After the examination, Dr Palmer said that any type of facial surgery was out of the question for me, and could possibly be a worse outcome. He suggested a filler to put volume back and he was so right. I am thrilled with the results. I feel that I look more youthful and feminine. He also suggested filler in my lips which Robyn injected, and I have never been so pleased. I had been to see numerous plastic surgeons for consultations, and all of them said I needed a complete face and neck revision to the tune of 25,000. Dr Palmer's words were " you can lay down. $40,000 in cash in front of me, and I still would not perform any surgery on your face" What a rarity.........to find a physcian who holds the patient's best interests as first and foremost.
