I have been in practice as a plastic surgeon since 1991. My practice has evolved from primarily reconstructive surgery to primarily cosmetic surgery. I am dedicated to patient care to provide optimal results. See the difference experience and skill makes.

Had a facelift and eye-lift with Dr. Kiener. I had researched and visited a total of 3 other Plastic Surgeons. On my first visit with Dr. Kiener and his staff they understood my wants. Didn't want to be like Joan Rivers. Wanted to have a fresh look instead of looking tired and get rid of my turkey neck. The office staff and the surgical team were awesome. I am so happy with the results.
Dr. Kiener is a consummate professional with a warm and caring bedside manner that inspires confidence - and for good reason! He and is staff make you feel cared for and respected throughout, and are readily available for questions or concerns. I'm thrilled with my results and very pleased to have chosen Dr. Kiener to perform the surgery.
So far everything has moved pretty smooth! I'm still waiting on covid results, so that makes me pretty nervous! I hope they come back in time. The staff I so nice! They are very accommodating, I don't feel like there is a question "too silly" to ask with them. The doctor comes across as very confident in his answers. This in my before photo. I'm measuring a b on one side b/c on the other. No upper pole after having kids. I want to fill in the upper pole and increase a D cup. My doctor has recommended 500cc HP unders. I'm pretty nervous they I'll be larger than I am looking for, but I will trust in the Doctor and the process, as he knows what he is doing! (As above, he seems very confident in these decisions!) Updated on 10 Jul 2020: The first 24 hours I was very nauseous and sore. Pain killers were only lasting for about an hour or so. I started alternating the pain killers an muscle relaxers to have one relief in between. The first night there was not a lot of sleep. Only an hour after after each round of pills. Only kept down fluids and soup (broth only) The next 12 hours were worse pain wise I tried not to move at all, but the nausea was finally gone! I kept down pudding, crackers, and chicken noodle soup. Showering helped relieve some tension in my back. My husband had to help me get undressed, dried off and dressed again. I did not leave recliner unless it was to go o the bathroom or change out ice packs. Once I made it to 36 hours things started to look up. I slept for 4 solid hours, only waking up to take my meds and change ice packs then right back to sleep. A hot shower felt AMAZING in the morning. I only needed help getting my second arm through my sleeve and the pressure band on. I'm now 8hrs without pain meds and I'm just now starting to feel tight and deep pressure again. I not feel gd need for a pain reliever, I'm only going to take a muscle relaxer and try with that, maybe a Tylenol. I've also been up walking around the house. I was able to drink coffee this morning, and I feel like I can try solid foods. I'll update in a few days. Oh, post op appointment the next morning I found out I got 500cc in left and 600cc in right. Updated on 10 Jul 2020: Not much has changed in last 24 hours. Still just sore. There was a weird suction feeling/sound on one side and now it looks a little lower! It was after I took today’s pictures, so we’ll see when I get undressed and shower tomorrow! The high part of my breast looks less pointed and more curved today! Still not taking pain meds during the day, but did wake up in pain in the middle of the night so I took one then. Lists of laying around still. Every thing tightens up more when moving around. Added prunes and probiotics in my diet. I don’t think I mentioned but I started stool softeners 2 days before surgery because I’ve had experience with constipation on pain meds. This round is no different.
Last year I finally reached my weight loss goals. Down 60 pounds, I had very deflated breasts, and a bit of sagging excess tummy skin. After a referral to Dr. Kiener from a friend, I scheduled a consult for a mommy makeover with him. After that visit, my husband and I decided to go ahead and schedule the surgery. Dr. Kiener did exactly what I asked... remove the skin, and make the breasts look great, but natural. I'm so happy with the results. The office staff is amazing. His nurse is the sweetest lady ever. There is almost never a wait for appointments. If you ever have an issue or question, they are very happy and quick to help.
I am currently about 8 weeks post-op and so far I am very pleased with my results! I am a 35 year old mother of two (breast fed), weighing in at 105 lbs. standing at approximately 5'3". I went from a size A to a size C cup. My implants are Mentor High Profile, Round, Smooth, 375 CC Silicone Gel, under the muscle with a transaxillary incision method. The procedure in total cost $6,328.00. I apologize I do not have very good photos. I have always been fairly flat chested, but there was some tissue at least to work with until my second child, after I was done breast feeding him, I lost everything! My chest looked like that of an 11 year old boy. My husband and I decided we are not having anymore children, and the opportunity arose financially, so I decided to pursue the surgery. I did a lot of research, looked at so many before and after photos. Real Self was an awesome tool that helped me reach my decision, as well as scored me a free consultation with Dr. Kiener. I like Dr. Kiener not only because he had good photos and by far the most experience than any other surgeon in this area (28 years), his reviews were solid and I was looking for someone that would do an incision other than inframammary, I understand that is easiest for the surgeon, and has the lowest rate of capsular contracture, but I really did not want any scarring on my breast if avoidable, and Dr. Kiener seemed to favor the transaxillary method (armpit). So thanks to real self, and Dr. Kiener's cooperation I was granted a free consultation with Dr. Kiener. My experience was pleasant, his office has a very nice atmosphere, and the wait times are so ideal, I wish every doctor could offer that kind of service! He was very upfront, straight to the point about everything, pros and cons. He informed me at this visit, because of my petite frame, that we would be looking at High Profile implants, because of their narrow base. I was concerned about this because I did not want to look super fake, or over-exaggerated, and then when he suggested the 375 CC, which with the sizers seemed really big for me, but Dr. Kiener and his staff reassured me that the high profile 375 CC would be the most ideal aesthetically and medically. And even though I had very little tissue to work with, because we were going under the muscle and the fact that I have had children made accommodating that size more feasible. I also went with silicone gel instead of salines because of the warranty that accompanied the silicone vs saline, as well as the fact that the silicone offered a more natural look and feel, which was very important to me. I feel the silicone were well worth the extra money! The surgery went well, I felt well cared for at the surgery center, excellent quality! As far as the pain goes, some women say they can get by with tylenol and no narcotics, but I found that to be impossible, if it were not for the muscle relaxers and percs I would have lost my mind because of the burning, pressure and tightness is almost unbearable, and it was this way for me for the first 5 to 7 days. Pain became more bearable around the 2nd week and I began to wean myself off the prescription meds, which about 4 1/2 weeks post-op I was able to get by without anymore prescription strength. I don't know if it is just my personal pain tolerance, but also the fact that I was stretched pretty far with the high profile and 375 CC may have made the recovery a bit more strenuous compared to women who may have had much more modest implant sizes and more tissue to work with. ?? Dropping and fluffing is stressful because all I did was worry that I was going to heal well and with the most ideal results. I religiously began my massage and exercises they recommend as soon as they said to. My left breast dropped and softened before the right breast, so I was very worried for a minute, but was reassured by other patients as well as my surgeon this was normal, especially because my right side is my dominant side so it fought a bit longer than my weaker left. The Doctor showed me some additional lower pole massages I could do to assist the implant in dropping into place, and that seemed to do the trick. By the 5th week my breasts were starting to look like normal breasts. And by 6 weeks, they were not only ideally positioned and soft but look amazing as well. My husband is just as satisfied with the results, and after being a little apprehensive because we were aware of the many risks, but the results this far have been better than what we had hoped for. I went from hiding my upper body with extra padding and deceptive clothing to wishing I could legally walk around topless, as they look that good! Dr. Kiener has the experience and it definitely shows. It would be my suggestion to go with what he suggests because he knows what he is talking about, definitely an artistic eye for aesthetics along with amazing medical skill/practice, so I would trust his opinion. For the money and what you have to go through, the last thing you want is to regret this decision. I was very fortunate in my decision with Dr. Kiener, and I hope that anyone looking in the Reno area will give him priority in consideration.
For over ten years, I've wanted to get a facelift to eliminate my jowls and saggy skin on my neck. The only reason why I didn't have this surgery done sooner was because I thought I would have to go back to California to find a top ranked doctor and it was never really feasible for me to take 2 to 3 weeks off and stay at a hotel, which didn't appeal to me at all! I researched local doctors, then met with Dr. Kiener and am so happy I did!!! Upon Dr. Kiener's assessment, he explained to me what he recommended and why. Also, he stressed to me that his goal is to not make one look like they've had a facelift. It was evident to me that he takes pride in achieving that goal and that"s very important to him. Right then, I knew I wanted him as my doctor. The surgery was at his nearby surgery center and everything went really well. As I healed, which was very quickly, I had numerous follow up visits for approximately 6 weeks and the results are fantastic! I feel like 15 years has been taken away and my confidence has been greatly enhanced! I can't say enough great things about Dr. Kiener and the time and commitment he has to ensuring my happiness. Everything turned out better than expected. He has the utmost integrity and I felt that he had my best interest at heart at all times. I had such a great experience I plan to have Dr. Kiener as my plastic surgeon forever!! P.S. I hope this helps you ladies! :)
Dr. Kiener did EXACTLY what I asked him to do -Tummy liposuction. The results EXCEEDED my expectations!! He has a wonderful manner and an outstanding staff. I had no down time and just a little bit of discomfort. I had gone to a another plastic surgeon and inquired about the same procedure and he recommended a TUMMY TUCK !!!! no thank you. I'm so glad I went to Dr. Kiener - thank you Dr. Kierner and your Staff. I highly recommend him.
This is my second procedure with Dr. Kiener and I couldn't be happier. I highly recommend him. The office staff are wonderful never any wait times. Before having any procedures done go see Dr. Kiener!! AH
Dr kiener is GREAT! He's straight to the point. Gives you the info you ask for, helps you decide the best plan for what your looking for! The entire office is personable and sweet! My breast augmentation came out amazing and was exactly what I was looking for! He used the armpit as the surgery point and I am so happy he did! Barely a scar you would never guess! He took any of my concerns into account and reassured me through the entire process! I will definitely recommend dr kiener and his staff to everyone!!
I’m a 30 year old mother of four (all breast fed babies). My breasts needed to be reinflated to say the least. I wanted to achieve a more natural look. Dr.Keiner did an amazing job of answering all my questions and addressing all of my concerns. Dr.Keiner has an eye for symmetry and balance. I was nervous about size and he was spot on with what would balance my body. He, his staff and the surgery center he uses are all exemplary. The surgery itself couldn’t have gone smoother. Everyone was kind and attentive. The surgery center even called everyday for three days post op to check up on me. Dr.Keiner’s aftercare is second to none. I am four weeks post op as of tomorrow and I have already seen him 4 times after my surgery. His office is always immaculate and he has always been punctual, and most of the time early for all of my appointments. I could not be more satisfied with my experience and the quality of care I received. Two of my own clients have already set up consultations with Dr. Keiner after seeing my results. I wouldn’t recommend anyone else. He is by far the best in the area.
Fat cells from adults may increase or decrease in size based on caloric intake /metabolism If you lose weight following fat grafting you may lose volume. The loss of volume following fat transfer is more a function of cell loss since the take or survival of those cells is not 100%. The biology that produced loss of facial fat and volume continues abated. In other words repeat fat grafting is very common. Regards.
The reason your jowls are worse post-op is probably due to swelling. Facelifts can alter lymphatic drainage of the face and may take up to 6 months to normalize. This means that they will improve with time. Some surgeons can improve jowls with a lateral smasectomy, but for me the best results are achieved with a supra-zygomatic SMAS dissection and a strong vertical vector. RegardsDr Kiener
The occasions are rare when I do not do a playsmaplasty/neck lift as part of a SMAS facelift. I am also oftentimes surprised at the amount of redundant skin that is apparent, following undermining , in necks that otherwise appear youthful. Based on your photo I would include neck lift as part of your facial rejuvenation.
Based on your photos I think you have an excellent result. Reducing your body fat will result in more definition, but may be difficult to maintain. Regards.Dr Kiener
You have accurately described your issues. A boxy tip caused by the shape and position of your alar cartilages, as well as a small dorsal hump. I think you are an excellent candidate for a rhinoplasty. Regards
