Updated on 26 Jan 2025: I came to Dr. James Nachbar for breast implant revision surgery with high hopes after a previous procedure. While the size adjustment was achieved, I have been left with noticeable asymmetry, including misaligned nipples, uneven pocket sizes, and a lack of overall balance and sculpted shape. One breast appears tighter and well-contoured, while the other looks softer, less defined, and droopy, with excess laxity that creates an unbalanced and uneven appearance. These issues are especially apparent when wearing thinner bras or fitted clothing, where the misaligned nipples and asymmetry stand out. I raised these concerns during my initial consultation, particularly about pocket size, laxity, and the possibility of needing a lift. Dr. Nachbar assured me that none of these would be an issue. Now, despite spending $13,000 on the procedure, I am being asked to pay an additional $4,000 for a revision surgery that is estimated to take less than an hour. However, even with the proposed revision, my concerns about pocket size, skin laxity, and overall contour are still not being addressed in his plan. This leaves me uncertain that the recommended solution will resolve the core issues. When I’ve expressed dissatisfaction, I’ve been met with defensiveness and dismissiveness, which has left me feeling undervalued as a patient. A good surgeon-patient relationship should be built on trust, respect, and open communication, and unfortunately, I’ve not felt that has been the case here. I understand that perfect symmetry can’t always be achieved and that surgical outcomes can vary. However, after investing so much time, money, and trust in this process, I expected a more thoughtful and collaborative approach to resolving my concerns, as well as greater transparency about revision policies and costs. If you are considering Dr. Nachbar, I recommend being very clear about your expectations, voicing all concerns upfront, and asking detailed questions about revision policies before committing. While Dr. Nachbar has expertise in certain areas, my personal experience has left me feeling disappointed and hesitant to move forward with additional work. Additionally, I referred three potential patients to Dr. Nachbar, one of whom spent $30,000 on services. Given my loyalty and the referrals I’ve provided, I would have appreciated a more accommodating approach, such as considering a breast correction as a professional courtesy.
Amazing!! My results are just what I had hoped for. I am only 4 weeks out and already love how I look and feel. I was back to work 2 weeks after my procedures and did really well, 4 weeks now and I am resuming my workout routine and ready to go shopping for smaller sizes. The "circumferential" was key for me selecting Dr. Nachbar and I have no regrets. His confidence in this method was so reassuring and after comparing a typical tummy tuck to the circumferential, I knew Dr Nachbar was My Doctor. Dr Nachbar also performed a breast reduction and lift, my back pain is gone, and I look and feel lighter than ever.
Dr. Nachbar performed a drainless tummy tuck 360, muscle repair, breast lift, breast reduction and liposuction December 2021. He and his staff took the time to answer all my questions. All 3 written pages. The surgery went fantasticly. My pain was extremely high and needed extra morphine at the recovery center and extra hydrocodone at home. The pain meds froze my colon so I had to come off of them. Which was super challenging. Going straight Tylenol was a painful process. But Dr. Nachbar & his staff were there every step of the way to support me. I cannot imagine going through that with drains. I'm so happy it was drainless. My muscles were tighten REALLY well, but not so tight that I couldn't breathe. I instantly saw amazing results, despite my swelling. The staff helped me to find a better binder to use vs. the one from the surgery center. My healing slowed & it was b/c I ended up getting an infection in my right breast. Dr. Nachbar immediately sent me some antibiotics & my healing immediately sped up. Not only did my breast start healing right away, but my belly button finally started healing too. My swelling also improved with the antibiotics. I ended up with a second infection and the follow up round of antibiotics stamped it out once and for all. The scabs on my right breast, above my public line, and on my left and right hips were persistent. I had to "air dry" them all to ensure they healed properly. Thankfully I was able to work from home during this time. My final results were amazing! I had a second round of liposuction September 2022 and I couldn't be happier with my results. Just lipo is so much easier to recover from. Just 3 down days & back to work with no issues. I'd totally do it all again. I'm so happy I went with Dr. Nachbar. All I have to say are nothing but good things about Dr.Nachbar. He has been my surgeon for years, I’ve had 3 surgeries so far. My nose, breast augmentation, and a couple of days ago I had another breast augmentation with him.
Tho is the day of surgery, I got 480ccs implants and lipo on my stomach and lower back. I was very nervous going in but Dr Nachbars staff made me feel as comfortable as they possibly could. Dr Nachbar himself is a great and thoughtful doctor and it really reflects on his staff. Updated on 20 Jul 2021: This was the day of surgery after I woke up. I stayed overnight and had help from two nurses who were kind enough to take pictures for me. This doesn’t show much so I’ll post more once I get around to it. Updated on 24 Jul 2021: There is still a good amount of swelling where the liposuction was done but I feel like it's coming along well. I got the drains out this same day and feel better not having them in the way. I have a good amount of projection from the implants but my camera angles do not show it very well. I'll upload better pictures pretty soon. Brushing is going down but soreness is still there, I'm impatient and just want to be healed already!! :( Updated on 31 Jul 2021: Right buttocks is going through extreme pain close to severe muscle spasms. It was extremely tough recovery, they did do intramuscular implants so that may be the reason why.
My plastic surgery experience with Dr. Nachbar could not have been any better. It was perfect. The procedure was called a left breast reduction. I wanted my left breast to be the same size and shape as my right breast and that is exactly what I got! I am so happy with the results I cannot even put into words. I love Dr. Nachbar's staff and office. Everybody is so kind, patient and clean. I definitely made the right choice, choosing Dr. Nachbar.
FINALLY got this procedure done, here's are my results! Third day was looking insane super swollen, could barely walk. My one piece of advice to you ladies is make sure you get your prescription filled before you go into surgery, I saw quite a few girls on here who didn't get to fill theirs just like me and I really wish I had them as the pain starts to kick in a couple after the surgery when the anesthesia starts to wear off. It's been almost a week and I'm healing nicely, I will keep these photos updated. I'm loving the results you can see how drastic it is.. I could cry. If you're thinking about getting this DO IT. Over a decade ago I had Gortex implant surgery in Los Angles and have hated the hardness of the implant. I actually hated my lips for years! Like anything else, your mileage may vary, but I and my patients have been very happy with the no drain tummy tuck. I have been using the no drain technique for all of my tummy tucks since 2008, and I highly recommend it.I have only used the suture technique, and have not had any experience using the glue technique. The suture technique (the way I do it) probably adds about 30 - 45 minutes to the length of the procedure. I place up to 40 or 50 separate sutures to eliminate the space where the fluid would otherwise collect and cause a seroma (fluid collection). Another advantage of the sutures is that they tend to pull the skin flap downward, which we believe reduces the tension on the incision and should therefore reduce the thickness of the scar in comparison to what one would expect if there were more tension.Your comment about the drain being the most feared and dreaded aspect of tummy tuck recovery is exactly on target. You can avoid all of the discomfort of having the drain in place for several weeks, as well as the discomfort of removing the drain, by avoiding the drain altogether.But even more important is the safety aspect of the no drain procedure: the most serious complications from tummy tuck (as opposed to the most common complication, which is seroma) are related to blood clot formation, and one of the best ways to reduce your chance of getting a blood clot is to get moving immediately after surgery. Being able to avoid the drain helps with that, because it hurts less to move around after surgery when there is no drain.That is especially true when Exparil long-lasting (three or four days) anesthetic is also used to reduce the post-operative pain and reduce the need for post-operative opioid pain medication. Combining the no-drain technique with the Exparil is a great way to reduce post-operative pain, and I believe increase your ability to move after surgery and thus reduce your risk of blood clots.I have to strongly disagree with those who talk about seroma needing to be drained when the no-drain technique is performed with sutures. I have not seen the sutures pull out, and I do not see seromas following tummy tuck when the no drain technique has been used the way I use it. On the other hand, seroma is the most common complication of tummy tuck when a drain is relied upon to try to get the skin and fat to stick down to the muscle. As I explain in the video, when there are no sutures holding the fat down to the muscle, every time you move you tend to pull them apart. Although I have heard that some surgeons claim that they can remove the drain after only a week or so, all of the articles I have seen have shown that seroma occurs more often when the drain is removed that early, and even occurs when the drain is left a full three weeks.Now, it certainly is true that the no-drain technique, especially with sutures, is harder and takes longer than the ordinary drain technique, and there is definitely a learning curve to become proficient in the no-drain technique.As I mentioned, I do not have experience using the glue technique; although I looked at it when it came out (I had been using the suture no-drain technique for many years already when the glue came out), the studies that the glue company provides all seem to indicate that many of the patients who have the no drain technique with glue still get seromas that needed to be drained with a needle. Again, I do not have experience using the glue, but since I don't see seromas with the suture technique, I would not want to go backwards and adopt a new technique with a reported incidence of seroma formation higher than I am seeing using sutures.Please take a look at the video, where I discuss this further, but I strongly suggest that you give full consideration to having a no-drain tummy tuck if you are planning a tummy tuck.Good luck with whatever you decide!
There are several ways to close a labiaplasty, but when continuous sutures are used for the outer layer, some bumpiness is expected in the weeks following surgery. Regarding the sutures, you should definitely follow your surgeon's advice, and not pull them out unless your surgeon told you to. Although suture marks are always possible when any incision is closed, the bumpiness you are seeing this early is not suture marks, but rather some swelling shaped by the sutures.So, follow your surgeon's advice, but I don't see anything concerning in your photo. Sorry you are having trouble. Unfortunately, the situation you describe cannot be fully evaluated without a visit to your surgeon. That is especially so because you are having pain. Your best bet would be to see your own surgeon again, but if that is not possible, or you are not sure you trust what your surgeon is saying or just want to hear it from someone else, you may benefit from a second opinion from another surgeon. In any case, visit an experienced and competent labiaplasty surgeon (your own or another) soon.

Absorbable sutures are one of the great advancements of surgery, and modern semi-synthetic sutures can be significantly stronger and last significantly longer than old-fashioned "catgut" sutures did. Indeed, some absorbable sutures can have strength that lasts up to six months, and any absorbable suture will still be present even after it has lost its strength, until the body finally absorbs it.Obviously, I don't know what sutures were used in your surgery, but I use two layers of absorbable sutures when I perform labiaplasty, because I want the closure to be solid (especially important for the wedge-type labiaplasty that I usually perform), and don't want to put the patient through the discomfort of removing non-absorbable sutures, especially in that area.The deeper absorbable sutures generally retain some strength for four to six weeks, but can, indeed, take three months to completely dissolve. Usually, they can no longer be detected long before that, but I would expect that some of the suture material is still present for three months.Therefore, although I know nothing about how your procedure was done, I can still answer your question: I do believe that it is OK for sutures to take three months to completely absorb.Hope that helps!

I think the procedure note answers your question: Although the note is not completely clear, it appears that your surgeon did a circumareolar mastopexy, which only uses an incision around the areola. Unfortunately, one of the problems with that procedure is that it tends to stretch out and enlarge the areola. In an attempt to prevent that, surgeons doing that procedure will often place a permanent suture all the way around the areola, although usually it is placed at the outside of the areola, rather than right around the nipple, as its sounds you are describing. According to your note, a Gore-Tex suture was used for that purpose in your surgery.A permanent suture has to be used, because a dissolvable suture will allow the areola to enlarge once it dissolves. Although several types of permanent suture can be used, Gore-Tex (expanded PTFE, or "teflon") is a popular choice because it is strong and slides easily.The good news is that a Gore-Tex suture is relatively easy to remove because it slides so easily, but the not-so-good news is that, if you have the suture removed, the areola is likely to expand in size significantly. The same thing can occur if one of the suture breaks--one of the areolae can stretch much larger than the other.A more complete solution may be to remove the Gore-Tex sutures, and then do a standard "anchor" breast lift to control the size of the areola without a permanent suture.You will need to seek out your surgeon to see if I am reading the note as your surgeon meant it to be read, and to discuss your options.
