I was treated by Dr. Vial and Dr. Walling at Bend Breast Care and I honestly cannot speak highly enough about them. Facing a double mastectomy is overwhelming but both doctors were in consistent communication with me every step of the way. I was 40yo at diagnosis, with two young daughters and running a business in Bend, OR. I spoke with doctors all over the country and the results of Walling and Vial were hands down the best. My double mastectomy plan for early stage breast cancer included: 1) nipple delay, 2) nipple and skin sparring double mastectomy with expanders placed and 3) exchange to 340cc mentor boost implants. There were definitely times along the way where I was really concerned about the final outcome. I put my trust in Dr. Vial and his team and I honestly can’t even believe how incredible the result is. I would highly recommend Dr. Walling and Dr. Vial for anyone facing breast reconstruction. I don’t know how they do it, but the results speak for themselves.

I had a wonderful experience with Dr. Vial and his team. From the very first meeting to the final appointment a full year later, I always felt like my concerns were taken seriously. I have the BRCA1 gene and decided to have a prophylactic double mastectomy with reconstruction and fat grafting. Dr. Vial did an incredible job of assuaging my fears before, during and after surgery. Not to mention the results are incredible. Because we did fat grafting, he was able to take the fat from my hips and move it to the top part of my breasts to make them look more natural. But because he’s a thoughtful and careful surgeon, he also gave my hips and butt a nice shape in the process. I know several other people who have had this surgery done elsewhere, and none of their surgeons gave a thought to the area the fat was removed from. I say this just to point out how detail oriented Dr. Vial is. His whole team is wonderful. Anna always made me feel so seen and cared for, and everyone in the office is warm and kind. I have recommended his office to several people and will continue to do so! I’m so grateful to have had Dr. Vial and his team help me through this difficult year.

I went from a 36K to a 36D. 5lbs was removed!!! From initial consultation to my 8 week post op graduation, Dr. Vial and staff were simply the best. The surgery itself performed by Vial was an absolute work of magic. His presence was calming and friendly. The surgery was extensive — a lot had to be done. I was under for nearly 7 hours. "Sometimes you get lucky" ... 'Dr Vial'. ...or, sometimes you find a phenomenally talented surgeon. A world class doctor right here in Central Oregon. I was so lucky to have had the opportunity to have been a patient of Dr Vial. A little background info of my road to see Dr Vial. I had previously had implants placed in 2012, and then explantation surgery to remove them in 2022 at another office in town. The explantation experience was unpleasant at best, and the doctor was dismissive, uncaring, and had left me somewhat disfigured with recurrent seroma pockets in my chest wall which run the risk of systemic sepsis. I had researched for a while to find a skilled surgeon to help. Dr Vial came out on top of the list of all my research, and it wasn't incorrect. He is supremely good at his job. I think any negative feedback you might find comes from the fact that he does not accept every case (warranted). He is a consummate professional and an intense perfectionist. This is exactly what you want when you're looking for a surgeon. He spent a significant amount of time with me during my consult and pre-op to make sure I was well informed, educated for the upcoming procedure, and that I had realistic expectations of the potential outcome. There was a lot of scar tissue from the previous surgeries and there was no guarantee I wouldn't have esthetic issues from the scarring. There is a surgery suite in his office, and the staff are amazing. The surgery was more complicated than expected and took him an hour longer than planned. The results....amazing. I have never looked so good. He was always available if I had any concerns or questions. I did happen to dial what I thought was an answering service on the weekend to ask a question about a week out from surgery and was shocked when Dr Vial answered the phone. I was horrified to have bothered him on a Sunday morning, but he was pleasant and answered all the questions I had. I would not hesitate to refer anyone to see Dr Vial. He is by far the most talented cosmetic surgeon anywhere close, and far for that matter. If you're researching doctors. This is your guy.
This should not be a problem if it really is just a small drop. You want to make sure that there is not a cancer issue, which is unlikely. To make sure, I would have you get an ultrasound and a mammogram. If the flow was more than a drop you may consider getting a prolactin level. If this was elevated then a trip to the endocrinologist and a head MRI may be needed to rule out a rare prolactinoma. Otherwise, I would advise limiting nipple stimulation to reduce flow.
This is a great question that many patients struggle with. Making a decision to do a lift is a difficult one as you are accepting more scars. The answer depends on many variables. Based on your photos, you could avoid a lift with larger implants. With smaller implants a periareolar mastopexy would be a good option.
A small percentage of patients will lose nipple sensation with breast augmentation. If it has been a year since augmentation, you will likely not regain sensation. Removing the implant will not make the sensation come back.
You do appear to have a mild case of tuberous breasts. I would recommend a periareolar mastopexy, dropping the breast fold, scoring the breast tissue, and placing an implant.
Your scars look like hypertrophic scars. Difficult to tell with the photos. Keloids are tumors and are often confused with hypertrophic scars. Treatments for hypertrophic scars include Aldera, steroids, and low dose radiation. My choice is Aldera. I perform a test spot on an area out of view. There can be many complication with all of these options and you should make a plan with your plastic surgeon.
