Cartilage protruding after primary rhinoplasty 5 years ago, can it be removed? (Photo)

Hi pd951, thanks for the question and the photo. The picture does look like a piece of cartilage pushing out from the columella. It is common in rhinoplasty to add cartilage to the columella in order to obtain better structure of the nasal tip. It could also be that, as your nose has aged, the cartilage has deviated over time and you are left with progressively more visible cartilage in the columella. Either way, a rhinoplasty is an option to address this. Hope this answer helps, good luck in your journey.

Thanks for the question and the pictures. Based on your pictures, it looks like you have lost some of the support in the bridge of your nose that is normally supported by cartilage. In order to resolve this issue, you will need a revision rhinoplasty and will require replacement of the structure of your nose. Hope this answer helps, good luck on your journey.

Bony dorsal hump after rhinoplasty, when can I get a revision? (Photo)

Hi Manthan. Great question. The time between rhinoplasty procedures is largely dependent on the extent of the recent surgery and the surgeon's preference. Generally, it is important to wait for the nose to heal for approximately 8-9 months before discussing the need for a revision because there can still be significant swelling that far out which is liable to change. Can any doctors who are experts on caudal septal deviation share their possible approaches for this problem?

Thanks for your question. Caudal septal issues are among the most challenging that a rhinoplasty surgeon has to deal with. The reason being that there are a variety of reasons someone might have problems with their caudal septum (eg. intrinsically weak cartilage, overgrown cartilage, prior surgery resulting in distortion of the caudal septum, etc.). This is the main reason that you see so many different potential treatment options regarding the caudal septum. In my practice, I employ a variety of techniques (eg. caudal septal replacement grafts, caudal septal splinting, caudal septal resection and reconstruction) depending on the needs of the patient. The most important thing is that your surgeon uses an appropriate technique based on your anatomy in order to achieve the functional and aesthetic outcomes that you are looking for.

Thanks for your question and posting those photos. In my opinion, the most important aspect of any rhinoplasty is your opinion as the patient.

The two most important things for you to consider are:
1) Are there any aesthetic issues that you have with your nose?
2) Do you have any difficulty breathing through your nose?

If so, then a rhinoplasty could be helpful for you. With that being said, your photos do demonstrate some findings that patients commonly ask to have changed via rhinoplasty.

Which procedure would correct my nose tip deformity? (photos)

Thanks for your question and photos. Your nasal tip is asymmetric with a midline cleft due to the size and positioning of your lower lateral cartilages. You would benefit from increasing the definition of your tip in order to make your tip symmetrical and make it align with the dorsum of your nose. This could be achieved via correction of the lower lateral cartilages (via sutures and reinforcing the current structure with your own cartilage) without changing the dorsum of your nose.

Thanks for your question. I personally avoid fillers in the nose because of their risks and lack of long-lasting improvement. Based on your pictures, I believe that you would benefit from a revision rhinoplasty. The benefit of this over filler would be the fact that a revision rhinoplasty would provide permanent structure to your nose and maintain the proper form and function of your nose.

Thanks for your question. This is a really common question that patients come see me for on a regular basis. The most common cause of post-rhinoplasty nasal obstruction is that the nasal airway was not fully addressed during the initial surgery. This can be really frustrating for patients, since many patients never had any breathing problems to begin with. While it is difficult to say what you precisely would need to fix your nasal breathing - I do believe that it is possible to correct your nasal blockage. This would require a revision procedure where your nose is fixed from both an aesthetic (size and shape) and function (nasal breathing) perspective. I specialize in ensuring the best functional and aesthetic nose possible.

Knew how to reconstruct and give internal support to my nose

Truly an exceptional plastic surgeon and human being! I was in desperate need of a revision rhinoplasty surgery, internally my nasal passages were collapsing and I was finding it increasingly difficult to breath. Externally and physically my nose was collasping as well, visibly crooked and skin receding exposing my columella. I was not interested in taking a chance with other plastic surgeons. I needed the BEST in his field, someone who SPECIALIZES in not only the aesthetics but knew how to reconstruct and give internal support to my nose. When I found that he literally teaches other plastic surgeons around the world how to perform his advanced procedures, and when I consulted with 2 other highly respected Harvard educated plastic surgeons and THEY only recommend Dr. Toriumi as the leading expert in the field, my search was over. The process is slow (if you need reconstruction such as mine) and you must be dedicated to his follow up procedures. The first year you are only 30% recovered. I can tell you this man is dedicated to his craft and to his patients, I was in another state and could not make it back to Chicago. He just happen to be traveling through this particular state and was able to meet me for a follow up. He is surrounded by a truly dedicated and professional staff. Results Are Simply Outstanding

A nasal fracture decades ago left the bridge of my nose collapsed on one side. My breathing was affected as well. Despite wanting to have these issues fixed it took me a long time to connect with the right surgeon. I spoke with many doctors in NYC about my condition over the years and one of them flat out told me that he didn’t feel he could help me but knew who could absolutely do the work - Dr. Toriumi. It was hard for me to make the decision to have surgery in Chicago because of the distance from NY but everything about the surgery (rhinoplasty + septoplasty) worked out wonderfully. Dr. Toriumi was very confident that he could help me from our first conversation and he and his office maintain a very high level of professionalism; the care I received throughout the entire process was exceptional. Just over three months out I can breathe better than ever and my nasal bridge is no longer collapsed and quite honestly simply straight. After 3 long years (Feb 2019 - Mar 2021) of physical, emotional and financial set backs, all consequences of enduring 4 failed rhinoplasty revision surgeries with another surgeon in Los Angeles, I was left with a badly deformed and non-functioning nose. The bridge of my nose had fully collapsed for the 4th time, my nasal passages were completely obstructed and I had lost most of my columella due to the removal of necrotized tissue from the area during an emergency procedure (surgery #3). I Have Had an Amazing Experience

I have been seeing Dr. Toriumi for the last 21 years. He performed a revision rhinoplasty and upper and lower blephoplasty surgeries on me. I suffered from extensive scaring during my first surgery and really don’t heal well so he guided me through my surgeries always addressing my concerns. I had a really busy work schedule back then and his staff always accommodated my scheduling needs as I am about an hour away. Dr. Toriumi is a quiet man but he always chooses his words thoughtfully and he is tremendously compassionate. I wouldn’t think of seeing another surgeon. As I have aged I now find I may need some attention beyond Botox. The most telling commentary I can make is that I have brought both my daughters to see Dr. Toriumi.
