I had a breast reduction last year (28G to 28C cup) I am happy with the size but unhappy with the nipple placement. I cannot show any cleavage and the volume sits in the lower half of my breasts. When I wear tops I can't show any cleavage without the top of my areola peaking out. I am looking for options to increase cleavage and make my nipples appear lower if possible.

You have a good result but the way your breast settled over time resulted in a « bottoming out » look. However, not bad by any means. Nevertheless, esthetically, you would benefit from a revision mastopexy, not just to tighten your breast skin enveloppe but for the main purpose of « reconstructing » your inframammary fold (under -the-breast crease). Specifically, the goal of the reconstruction is to elevate your crease and stabilize it to the lining of the ribs above which would also « push up » your breast tissue. This will effectively decrease the amount of breast tissue lying below your nipple, giving your more superior fullness at the same time while lowering your nipples to a more normal position in relationship to the breast. It seems you had a vertical scar only technique for your surgery., where this can happen. The best option, at this time , is to have a revision/lift using the traditional Wise pattern.
