Hello CR88. This looks like a lot of skin in association with the extra breast tissue. Most likely - direct excision with a scar hidden in the armpit region. Best, Dr. ALDO.

From the photograph it looks as though there might be excess skin. If this is the case in a scars necessary direct excision is the best choice. Many times liposuction alone can be used if the skin can retract to the new reduced volume. This is a judgment call to be made by your plastic surgeon.
