Hi, A couple of years ago I bit my lip really hard and it has left me with white scar tissue when I smile. When I am not smiling I have a deep line where the scar is. Although I have other lines on my lip I feel this one shows as it is very deep. What can be done about this? Many thanks

Not all elevated, thickened scars are the same. Scars that grow beyond the borders of the initial injury (incsion, cut, etc) are referred to as keloids. They are not as common as hypertrophic scars but are more challenging to treat. Steroid injections can provide benefit but...
You have a very hypertrophiv irregular sternal incision scar. Best to seek in person care from steroid injections over a few months, possible re excision to radiation (doubtful).
Keloid therapy will take time. You may wish to have him involved in future treatments in the future. It is not a disease, so you should feel comfortable in discussing the future therapies.
