I have been very pleased with Sculptra as a way to fill out buttock indentations. I find that the procedure requires little to no downtime and is office based. I have done minor dimples to full buttock augmentations, albeit the latter is for people who want a modest augmentation and with little fat to donate. At times, if the indentation is associated with a scar, then subcision or a scar release may also be warranted. Alternatively, fat grafting is an option.

Dimples of the buttocks and thighs are a very common problem for women of all ages, ethnicities, and body types. These dimples are referred to as cellulite, and they are a result of the tethering of the skin by fibrous bands that course through the fat in the buttocks and thighs. For decades different treatments were tried and found never to be effective for long lasting correction of cellulite. There finally is a new treatment called Cellfina that creates long lasting correction (2-3 years) of this problem. The Cellfina treatment is performed under local anesthesia. The fibrous bands are cut with an oscillating microblade, and the skin becomes significantly smoother! A contour irregularity or dimple of the buttocks can be congenital or have a variety of causes. There may have been a history of injections in the buttocks as a child (a common delivery method for antibiotics or vaccinations) which may have resulted in shrinking of some of the fatty tissue and scar build up under the skin leading to the indentation. A history of trauma of this area can lead to the same problem. If the dimple is isolated, it can be corrected by undermining it to loosen the dimple from the deep attachements and then fat grafted to help "pop" it out.
