Hello Doctors! About 8 months ago I got direct lip lift surgery, the incision of the lift was directly on my lip. After I did this procedure to reduce my philtrum it caused my philtrum to protrude a lot and caused my mouth to look like a gorilla when I smile. How can I fix this problem? I do not like my smile anymore and would like to flatten my philtrum.

A small corner lip lift would help elevate the corners of the mouth. There is a incision right at the Vermilion border so sometimes it can be seen. A small amount of your own fat to the corners would also enhance the upward tilt of your corner of your smile.
While is good to be concerned after four rhinoplasties likely the skin of the lip is undamaged. The skin right below the nose may be a little bit thinned out, but that skin ultimately will be excised by doing the lip lif.t so a conservative lip lift in this case will probably help and the skin...
Yes, you can still get a very conservative lip lift approximately 4 mm. With 4 mm of excision there will be no elevation of your incisive show. I would also recommend tiny bit of fat grafting as well laterally and medially to give you a little more fuller lip without causing any more incisive show.
