Is it possible for the submuscular pocket to be too small?

I had little breast tissue before i underwent the surgery (AA Cup), My surgeon and i decided to do 325/300cc Submuscular. After i woke up, he told me he couldn't make a pocket big enough Submuscular. So he placed my implants Subglandular with much smaller implants 240/215cc. my question; is it possible that a surgeon can't make a pocket big enough Submuscular for the breast implants? I am very confused at this point and i am looking for revision now
