42 years old, had LASIK 6 weeks ago for myopia. Non-dominant eye was slightly overcorrected, about ~+0.25, the dominant eye is slightly undercorrected, ~-0.25. I have noticed when I work on the computer I can read fine, but when I look up at a distant object my dominant eye is extremely blurry, non-dominant eye has fine vision. Blurriness goes away after 5-10 minutes, but eye feels strained. I can still read small print fine. The dominant eye has been really dry since surgery. Cause of this?

You need to take drops for a few months after PRK to prevent scarring, if you stop them too soon you may scar

Both of the lasik systems that you list are excellent. I would not base my opinion on which of these systems are being used. Rather, I would recommend that you look into the reputation of the doctor. This is by far the most important aspect.
Although LASIK can be performed off label, I wouldn't do it for a 13 y.o. My main concern is that younger people, especially those under 18 years of age, have very placid corneas. Placing and incision in this young of cornea while making the LASIK corneal flap puts the cornea at risk...
