Can spreader graft shift after 4 months?

Once again, the devil is in the details.  The fact that you are describing your recent nose  surgery with the terminology of surgical parlance is quite intriguing, and makes other surgeons wonder exactly why a spreader graft may have been needed, and even more begging the question as to  where exactly was the spreader graft placed, how it was secured surgically, what were the exact dimensions of the graft, and where did the cartilage come from ??  All that aside, at four months-most grafts would be completely stable with a very low likelihood of shifting-but once again the devil /details moniker-what type of physical manipulation are you alluding to??  Perhaps the light hand manipulation of some massage therapist, the deep punishing extraction of blackheads from your nasal tip.....or the blow from a Louisville slugger compliments of an opponent during a pick up baseball game??  Most accidental,  light handed touches to your nose should not shift any of the infrastructure at 4 months-other  manipulations involving up to deadly force-all bets are off !!  Good luck to you !!

The likelihood of movement of spreader grafts 4 mos after surgery is next to 0.  If they have been in place thus far, there has been enough scarring etc to keep them in place.

Depending on how the spreader grafts were secured it is extremely unlikely they will shift. But nothing in life is guaranteed.
