From your photo, I note relatively normal placement of the nipple-areolar complexes in the chest wall. The distance from the nipple to the fold beneath the breasts is slightly increased and the nipples reside well above the fold at the bottom of the breasts with minimal sag. There is a degree of loss of superior breast pole volume.
As your desire is a D cup, a 470 cc augmentation will provide a satisfactory result. If a smaller implant is chosen, a lift may required.

After I measure and discuss goals with my patients, we then look at pictures of women (my patients) with similar measurements with different size and style breast implants. You would probably do well without a lift. Your breasts naturally sit somewhat low on your chest and implants will not change this.
