Dr. Paul E. Chasan has been a board-certified plastic surgeon in Del Mar, California for nearly three decades. The founder of Ranch & Coast Plastic Surgery, Dr. Chasan specializes in facelifts, rhinoplasty, breast augmentation, and revisionary breast surgery. Using state-of-the-art surgical technologies and techniques, Dr. Chasan also regularly performs tummy tucks (abdominoplasty), liposuction, mommy makeovers, breast lifts, breast reductions, eyelid surgery, and other facial rejuvenation and body contouring procedures. Dr. Chasan is a former president of the San Diego Plastic Surgery Society. He is considered by many to be a “surgeon’s surgeon,” in that he often operates on other plastic surgeons and their family members. After earning his medical degree from Creighton University School of Medicine in Omaha, Nebraska, Dr. Chasan went on to complete his residency in General Surgery at the University of California San Diego, where he served as the Chief Resident during his final year. He finished his specialized training during a two-year residency in plastic surgery at the University of Utah in Salt Lake City. When he is not working at Ranch & Coast Plastic Surgery, Dr. Chasan regularly lectures to plastic surgery residents at the University of California, San Diego. He is also a past board member for Angel Faces, a nonprofit organization that provides education and ongoing support for girls and young women with burn and trauma injuries. Board certified by the American Board of Plastic Surgery, Dr. Chasan is a member of the American Society of Plastic Surgeons (ASPS) and a fellow of the American College of Surgeons (FACS).

A: Thank you for your question. A large portion of my practice is dedicated to breast revision surgery, and I have been in practice for over 28 years. The question is not whether to use Tigr mesh or Strattice, but who is your revisionary surgeon. You have a very difficult revisionary breast surgery and need someone very experienced and this type of work. The use of meshes and foreign bodies will not help in your situation. I would recommend a smaller implant, capsulorrhaphy and a vertical breast lift. You should be able to have an excellent outcome.

A: Thank you for your question. A large portion of my practice is dedicated to breast surgery and revision breast surgery. I have been in practice for over 28 years. In your case, you would benefit from an internal bra procedure - capsulorrhaphy. I wrote the very first paper on capsulorrhaphy over 20 years ago. If you are on top of the muscle, then I would reposition the implants under the muscle. I wrote that paper too. I rarely use mesh or feel that is needed in most cases. I find that many surgeons are not skilled at these procedures and have high rate of recurrence. You should be able to have an excellent result. Best wishes to you

A: Thank you for your question. A large portion of my practice is dedicated to breast surgery and revision breast surgery. You are an excellent candidate for a standard dual plane breast augmentation with silicone implants. I have a whole gallery of patient's I have long-term deformities associated with placing implants on top of the muscle. There is virtually no support when you place the implants on top of the muscle, and they tend to slide down and have a much higher rate of capsular contracture. An animation deformity is possible and probable in every body. The vast majority are happy and the animation deformity is minimal. You should be able to have an excellent result. Best wishes to you.

A: Thank you for your question. You are an excellent candidate for rhinoplasty, I think it is one of the most beautifying surgeries that we do as plastic surgeons. A large portion of my practice is dedicated to rhinoplasty surgery. It is important to only see plastic surgeons with extensive experience in rhinoplasty, ie perform at least 6 per month and have performed hundreds of cases. Also, critically evaluate on-line galleries to see if their art form resonates with yours. Rhinoplasty is definitely a grey haired plastic surgeon's sport. Best wishes to you.
