Q: Should I have lower eye surgery to repair a botched filler procedure? (Photos)

A: Dear lbruce8883, 

Bellafill is not an HA (hyaluronic acid) filler and cannot be dissolved with hyaluronidase - perhaps it was Belotero that was used?  In any case it can take a while (months) for HA fillers to dissipate, even with appropriate use of hyaluronidase.  Sometimes repeated hyaluronidase injections are helpful. The submitted photos show significant remaining mid-facial volume loss that can be addressed with a different injectable HA or with other options such as Sculptra injections or fat grafting. Certainly lower eyelid surgery is not the answer for your concerns.

Q: What is the cause for fat and volume loss after a BBL?

A: Dear AD17, 

BBL/IPL treatment should not cause significant changes in facial volume - at least this is not among the well known complications and side effects of this type of light treatment.  Certainly unusual outcomes can occur but loss of facial fat volume may more likely be the result of something else (e.g. radio frequency treatments, metabolic changes).  Elevated temperature certainly does induce fat emulsification and fat cell destruction (lipolysis) but both the temperature and length of time required for this to happen are not achieved during typical BBL/IPL treatment sessions.

A: Dear bee1990, 

Lower eyelid surgery to reposition the outer (lateral) part of the eyelid more superiorly (lateral canthoplasty or lateral tarsal strip procedure) may be the appropriate procedure for you but a formal consultation with physical examination is needed for this determination.  Lateral canthoplasty may make the eye opening (palpebral fissure) noticeably smaller in patients with upper eyelid drooping (ptosis) even if mild...

The injector's technique and choice of product do make a difference here. It is important to distinguish where the volume loss is anatomically – mostly at and above the orbital rim, mostly below the orbital rim, or both.  For patients with slight hollowing at/above the orbital rim a softer product such as Belotero Balance is ideal while for patients with deeper hollowing below the orbital rim (this is the mid face) a heavier product such as Voluma is much more effective.

A: Thank you for your question.

Creating the “chemical brow lift” effect with neuromodulator injections (e.g. Botox Cosmetic, Dysport) is not possible for all patients.  The chemical brow lift occurs when the balance between brow depressor and brow elevator muscles is shifted in favor of the latter.  This works best in patients with very little actual ptosis (sagging) of the brows but both hyperactive brow depressor muscles and reasonably strong brow elevator muscles.  The key to success is appropriately reducing the hyperactive brow depressor muscle activity while also preserving enough brow elevator muscle activity.  So this means adequate injections of the eleven line area (procerus and corrugators), lateral brows and upper crows feet (orbicularis) and, if the horizontal forehead lines are to be targeted, it is important to preserve the ability to elevate the brows.
