Dont feel embarrased to ask him anything you like. Call the office and they will give you a email to send your pictures to. Then you can have the phone consultation and ask away. With a distance so far away like CA, I think you should fly.

He wnt give you shots. They are illegal and unsafe. Is your life really worth it?
